Cardiac Mri in Cardiomyopathies

Abstract: Heart failure affects 1–2% of the adult population and one of the main contributors to its development is cardiomyopathy. Assessing a patient's risk for adverse events in heart failure is challenging and made more difficult by the heterogenous phenotypic expression of the disease. Cardiac MRI has long been a gold standard measure of myocardial function and anatomy due to its high spatial and temporal resolution. More recently, it has been posited to play a more critical role in the diagnosis and prognosis of c… Show more

“…This resulted in an overestimation of LVEF of 6.6 ± 8.3% by echocardiography compared with CMR (echocardiographic LVEF: 31.5 ± 8.7% and CMR LVEF 24.9 ± 9.6%) [30]. Similar findings were reported in a 2022 review by Lahoti et al, which estimated the mean difference in LVEF measurement between echo and CMR to be 4%, with LVEF as measured by CMR being more predictive of mortality [31].…”

“…HF is caused by a loss of functional myocardial cells after injury to the heart from various causes. The most common etiologies are broadly classified as ischemic or non-ischemic cardiomyopathies [31]. According to epidemiological studies and large-scale treatment trials, patients with ischemic HF have a worse prognosis than those with non-ischemic etiologies.…”

“…In addition, it is possible to derive multiple types of functional information. Cardiovascular MRI of the heart is used to determine endsystolic-, enddiastolic volumes, stroke volume, ejection fraction and multiple other parameters ( 120 ). Brain activity is associated with changes in blood flow and the amount of oxy- and desoxyhemoglobin and these parameters are used by functional MRI (fMRI) to gain information about brain activity ( 121 ).…”

“…Endomyocardial biopsy (EMB) remains the gold standard for diagnostic confirmation of inflammatory cardiomyopathy. Cardiac magnetic resonance imaging (CMR) is the non-invasive gold standard, which may also be used to identify affected myocardial regions for EMB ( 8 ). The availability of both methods is limited due to infrastructure and expertise concerns across medical institutions.…”
