Cardiac resynchronization therapy outcomes in patients under nonoptimal medical therapy

Abstract: BackgroundCardiac resynchronization therapy (CRT) is indicated in symptomatic heart failure (HF) patients after achieving optimal medical therapy (OMT). However, many patients may not be under OMT when the CRT device is implanted. Here, we evaluate the long‐term benefits of CRT in symptomatic HF patients receiving or not OMT.MethodsWe investigated the effect of OMT on HF developing or death in 328 consecutive patients with a CRT device implanted between 2005 and 2015 in a single tertiary center. After the CRT … Show more

“…When we separately analyze CRT responders and non-responders, we observe that during follow-up there is a decrease in heart rate in all HF patients, that it is statistically significant only in CRT responders. The reasons for this result are related to the intensification of antiarrhythmic therapy in all patients after CRT implantation, as well as to the improvement in clinical status the recovery of vagal tone, which dominantly determines this parameter, in patients who benefited from device implantation [36,37].…”

“…В последние десятилетия достигнут серьезный прогресс в ведении пациентов с ХСН со сниженной сократимостью миокарда ЛЖ. Современная медикаментозная терапия, дополнительные средства (кардиовертер-дефибриллятор, ресинхонизирующая терапия) позволяет улучшить прогноз в этой категории больных [2,3]. Вместе с тем, эта разница несколько нивелируется у пациентов старших возрастных групп [4].…”
