Cardiac Surgery in Patients with Acute Intermittent Porphyria

Abstract: Acute intermittent porphyria (AIP) is a disorder of the heme biosynthesis pathway of a specific inherited enzymatic defect. Many different environmental factors such as drugs, calorie restriction, infection etc., often play a key role in triggering a porphiric acute attack. We describe a 50-year-old woman with AIP who underwent aortic valve replacement due to acute infective endocarditis and review the perioperative management of these patients.

“…Acute intermittent porphyria (AIP) is an autosomal dominant metabolic disease caused by gene mutations that results in decreasing heme synthase activity and accumulation of metabolic substrates, leading to neurological dysfunction (1). Primary Sjögren's syndrome (PSS) is a chronic inflammatory autoimmune disease that mainly affects exocrine glands such as the lacrimal and salivary glands (2) and that has a definite familial aggregation.…”

“…Its inheritance is autosomal dominant. [ 1 ] A deficiency of porphobilinogen deaminase is not sufficient by its self to produce AIP, and other activating factors must also be present. These include some drugs, hormones, infection, injury, stress, dehydration, fasting, puberty, and alcohol.…”
