Cardiovascular events and all-cause mortality in surgically or medically treated primary aldosteronism: A Meta-analysis

Abstract: Objectives: To compare the effect of surgical or medical treatment on the risk of cardiovascular diseases (CVD) and all-cause mortality in patients with established primary aldosteronism (PA). Methods: We searched PUBMED, MEDLINE and Cochrane Library for the meta-analysis. We included patients who were diagnosed with PA following guideline-supported protocols and received surgery or mineralocorticoid receptor antagonist (MRA)-based medical treatment, and age-sex matched patients with treated essential hyperten… Show more

“…[1][2][3][4] Unilateral PA (UPA), which is treated by adrenalectomy, and bilateral PA (BPA), which is treated medically, are the 2 major subtypes of PA. Patients with surgically treated PA have better long-term outcomes compared with patients with medically treated PA [5][6][7][8][9][10] ; hence, accurate subtyping is important. The current guidelines recommend adrenal venous sampling (AVS) for subtyping in most patients with PA. [11][12][13] Corticotropin (ACTH) is used during AVS in some centers due to reported benefit of increasing the rates of successful adrenal vein cannulation.…”

“…In line with the metabolomic traits, the MI in patients with PA, such as citrulline/ornithine and ornithine/arginine, which are associated with increased cardiovascular risk and phenotypic features of metabolic syndrome, were mainly associated with cortisol, cortisone, corticosterone, and DHEA in our regression analysis. Based on these observations, it can be speculated that the poorer cardiovascular outcomes in patients with PA treated pharmacologically by inhibiting aldosterone activity compared to surgery ( 52 ) might be related to the unopposed activity of the other adrenal steroids.…”

“…In contrast, surgical adrenalectomy may reduce aldosterone levels to normal in unilateral hyper-aldosterone-producing adenoma. Adrenalectomy can cure hypertension among 30-60% of patients and completely correct hypokalemia, resulting in a decreased incidence of atrial fibrillation and cardiovascular-related mortality in PA patients [24,25].…”
