In the beginning of COVID-19, the proportion of confirmed cases in the pediatric population was relatively small and there was an opinion that children often had a mild or asymptomatic course of infection. Our understanding of the immune response, diagnosis and treatment of COVID-19 is highly oriented towards the adult population. At the same time, despite the fact that COVID-19 in children usually occurs in a mild form, there is an incomplete understanding of the course as an acute infection and its subsequent manifestations such as Long-COVID-19 or Post-COVID-19, PASC in the pediatric population, correlations with comorbidities and immunological changes. In mild COVID-19 in childhood, some authors explain the absence of population decreasing T and B lymphocytes. Regardless of the patient’s condition, they can have the second phase, related to the exacerbation of inflammation in the heart tissue even if the viral infection was completely eliminated—post infectious myocarditis. Mechanism of myocardial dysfunction development in MIS-C are not fully understood. It is known that various immunocompetent cells, including both resident inflammatory cells of peripheral tissues (for example macrophages, dendritic cells, resident memory T-lymphocytes and so on) and also circulating in the peripheral blood immune cells play an important role in the immunopathogenesis of myocarditis. It is expected that hyperproduction of interferons and the enhanced cytokine response of T cells 1 and 2 types contribute to dysfunction of the myocardium. However, the role of Th1 in the pathogenesis of myocarditis remains highly controversial. At the same time, the clinical manifestations and mechanisms of damage, including the heart, both against the background and after COVID-19, in children differ from adults. Further studies are needed to evaluate whether transient or persistent cardiac complications are associated with long-term adverse cardiac events.

BackgroundMultisystem inflammatory syndrome in children (MIS‐C) is a rare and serious systemic inflammatory disorder that occurs following a severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) infection. This study aims to investigate the clinical manifestations, risk factors associated with pediatric intensive care unit (PICU) admission, and outcome among children with MIS‐C in Taiwan.MethodsA retrospective analysis was conducted among pediatric patients diagnosed with MIS‐C between June 2022 and February 2023 at Chang Gung Memorial Hospital, Linkou, Taiwan. Data on demographics, clinical features, laboratory findings, treatment modalities, and outcomes were collected and analyzed.ResultsTwenty‐eight MIS‐C patients, including 9 boys and 19 girls, with an average age of 5.3 ± 3.8 years old, were enrolled. Most of the cases (78.6%) were diagnosed following the first pandemic wave of COVID‐19 in Taiwan. The leading clinical manifestations observed were fever (100%), skin rash (64.3%), tachycardia (46.4%), and vomiting (46.4%). Nine patients (32.1%) were admitted to the PICU due to hypotension or neurological manifestations. Higher levels of band‐form white blood cells, procalcitonin, ferritin, d‐dimer, prothrombin time, NT‐proBNP, and lower platelet levels on arrival were associated with PICU admission (p = 3.9 × 10−2,9 × 10−3, 4 × 10−3,1 × 10−3, 5 × 10−3, 4.1 × 10−2, and 3.4 × 10−2, respectively). Arrhythmia in one case (3.5%) and coronary artery abnormalities, including dilatation in two cases (7.1%) and small aneurysms in one case (3.5%) were identified. Regardless of ICU admission, no patients experienced systolic dysfunction or mortality following treatment.ConclusionMIS‐C cases in Taiwan have a favorable outcome. Although one‐third of the patients required PICU admission, none of the MIS‐C cases resulted in severe cardiovascular morbidity or mortality. This study provides valuable insights into the clinical manifestations and outcomes associated with PICU admission in children with MIS‐C in Taiwan.

Đặt vấn đề: Hội chứng viêm đa cơ quan ở trẻ em (MIS-C) là một tình trạng bệnh lý hiếm gặp nhưng nghiêm trọng, thường xảy ra từ 2–6 tuần sau khi nhiễm SARS-CoV-2. MIS-C được đặc trưng bởi tình trạng viêm ở các hệ thống cơ quan khác nhau, chẳng hạn như hệ thống tim mạch, hô hấp, niêm mạc và đường tiêu hóa, nguyên nhân của MIS-C vẫn chưa được biết rõ. Mặc dù MIS-C là vấn đề nghiêm trọng và việc nghiên cứu về yếu tố dịch tễ, đặc điểm lâm sàng và cận lâm sàng giúp cung cấp các số liệu và nâng cao hiệu quả điều trị. Mục tiêu nghiên cứu: Nghiên cứu yếu tố dịch tễ, đặc điểm lâm sàng, cận lâm sàng của Hội chứng viêm đa cơ quan ở trẻ em nhiễm SARS-CoV-2 (MIS – C) tại Bệnh viện Nhi đồng Thành phố Cần Thơ. Đối tượng và phương pháp nghiên cứu: mô tả hồi cứu 58 bệnh nhân được chẩn đoán là Hội chứng viêm đa cơ quan ở trẻ em nhiễm SARS-CoV-2 (MIS - C) được điều trị tại Bệnh viện Nhi Đồng Cần Thơ trong thời gian từ tháng 09/2021 đến tháng 09/2022. Kết quả: bệnh nhân có độ tuổi trung bình là 5,47 (2 tháng - 13 tuổi) và 69% trường hợp là nam, thời gian mắc COVID-19 đến khi khởi phát triệu chứng có liên quan đến MIS-C trung bình là 6,63 tuần với thời gian nằm viện trung bình là 11 ngày. Tất cả các bệnh nhân đều có sốt với thời gian sốt trung bình là 7,14 ngày. Phát ban xuất hiện ở 81% các trường hợp, thay đổi niêm mạc với triệu chứng lưỡi dâu chiếm 34,5%, môi đỏ với 63,8%, bệnh nhân tiêu lỏng chiếm 27,6%, có 28/58 bệnh nhân bị phù chi (41,4%), nổi hạch ở cổ chiếm 8,6%, ghi nhận có 10/58 bệnh nhân bị sốc (17,2%). Bệnh nhân có tăng bạch cầu chiếm 48,3%, tiểu cầu tăng chiếm 10,3%, có 51 bệnh nhân được làm xét nghiệm CRP với giá trị trung bình là 122,9 mg/L, Pro-calcitonin có 36 đối tượng được thực hiện xét nghiệm nồng độ với giá trị trung bình là 34,31 ng/mL, ghi nhận 45 bệnh nhân được làm Ferritin có giá trị trung bình là 411000 µ/L, giá trị D-dimmer đo được ở 49 bệnh nhân với giá trị trung bình là 7,05 µg/ml, Fibrinogen được thực hiện ở 52 trường hợp có giá trị trung bình là 5,29 g/L, siêu âm tim ở 55 bệnh nhân thì có 13/55 trường hợp có giãn mạch vành (22,4%) bao gồm 7 ca giãn động mạch vành trái và 6 ca giãn cả hai động mạch vành. Chúng tôi nhận thấy có sự khác biệt về bệnh nhân bị sốc với nồng độ Ferritin (p=0,037), sự khác biệt giữa giãn động mạch vành với số lượng tiểu cầu (p=0,018) và sự tương quan giữa nồng độ Procalcitonin với số ngày nằm viện (0,029). Các phát hiện ở miệng hoặc hầu họng có liên quan đáng kể với sự hiện diện của phát ban toàn thân (p=0,008) và viêm kết mạc (p=0,001). Kết luận: Tổn thương cơ quan thường gặp nhất trong MIS-C là sự thay đổi về niêm mạc da. Nồng độ Ferritin càng cao làm gia tăng nguy cơ sốc và số lượng tiểu cầu cao làm tăng khả năng giãn động mạch vành ở bệnh nhân MIS-C. Bệnh nhân có nồng độ Procalcitonin cao khi nhập viện được phát hiện ở lại bệnh viện lâu hơn. Sự hiện diện của những thay đổi ở miệng hoặc hầu họng có thể là dấu hiệu sớm của MIS-C và nên được coi là gợi ý MIS-C trong bối cảnh nhiễm COVID-19 hiện nay.
