Care Transitions From Patient and Caregiver Perspectives

Abstract: Clear accountability, care continuity, and caring attitudes across the care continuum are important outcomes for patients and caregivers. When these outcomes are achieved, care is perceived as excellent and trustworthy. Otherwise, the care transition is experienced as transactional and unsafe, and leaves patients and caregivers feeling abandoned by the health care system.

“…Overall, caregivers in our sample were inquisitive and proactive learners. Feeling prepared to manage the patient's anticipated care needs is important for both caregivers and patients experiencing hospital-to-home transitions (11). Although many caregiver participants had existing knowledge and experience of the care recipients' PF needs, for some, declines in PF after a hospitalization presented new challenges.…”

“…The perceived burden of care not only affects the caregiver's wellbeing but also causes the caregiver to experience stress when performing his/her caregiving role . For caregivers, stress is directly related to the way that caregivers mentally process the care that they provide . Given that stressors have direct effects on a caregiver's health, it is important to plan for interventions that can determine the caregiver's perception of the burden of care and his/her stress level and that can mitigate the stress affecting them …”

“…9,10 For caregivers, stress is directly related to the way that caregivers mentally process the care that they provide. [11][12][13] Given that stressors have direct effects on a caregiver's health, it is important to plan for interventions that can determine the caregiver's perception of the burden of care and his/her stress level and that can mitigate the stress affecting them. 12,14 In the case of diseases involving the nervous system, if nurses pay holistic attention to individuals who assume the caregiving role and examine the relationship among the caregiver burden, caregiver wellbeing, and the caregiver stress level in these individuals, this will positively affect both caregivers and patients and highlight the importance of preserving the wellbeing of the caregivers.…”

“…37 Like us, others have noted how kindness and caring by the care team in hospital can affect the transition from hospital to home. 38 Our study highlights the limits of the coverage of Ontario's public health insurance and how these conflict with patient and caregiver expectations. Policy experts have long noted that Canada has deep public coverage for a narrow basket of services, specifically hospital, diagnostic and physician services.…”
