Carotid-anterior cerebral artery anastomosis

Abstract: The angiographic and anatomical features of an anomalous communication between the intradural internal carotid artery and the anterior cerebral artery are described. Essential features of the anastomosis include an origin at, or close to, the origin of the ophthalmic artery, a course ventral to the ipsilateral optic nerve and anterior to the optic chiasm, and a termination near the anterior communicating artery. Although rare, the condition should be considered as an entity. The incidence of associated berry a… Show more

“…The most common site of aneurysm formation is the ACA-AcomA complex. 2,4,6,7,9,[11][12][13] However, aneurysm formation may occur anywhere within the circle of Willis, such as the proximal MCA 2,3,6) and BA 3) as seen in the present case. Hemodynamic factors are likely contributors to the pathogenesis of the associated cerebral aneurysm.…”

“…The artery then ascends medially to join normally positioned anterior communicating artery. Although a supraoptic A1 connection is not shown in our case, an associated normally positioned but extremely hypoplastic A1 has been reported 4,5 . Infraoptic A1 is unilateral in most cases with only three cases of a bilateral infraoptic course reported in the anatomic and neurosurgical literature 1 .…”

“…Certain anomalous arterial variations in the circle of Willis are known to have an associated increased incidence of cerebral aneurysms. Other associations include contralateral ICA agenesis 4 , common origin with an ophthalmic artery 1 , azygos anterior cerebral artery 6 , arteriovenous malformation 7 and aortic coarctation 1 . Our patient had an associated anterior communicating artery aneurysm and coarctation of the aorta.…”

“…23) Twenty-two aneurysms occurred in 15 of the 67 cases of PTA variant. 1,6,9,12,14, 19,20,26,28) Most of the associated aneurysms were found at the circle of Willis. Only four cases 9,20,39) including our case were associated with an aneurysm arising from the junction of the ICA and PTA variant.…”

“…36) Since then, 67 cases have been described. 1,4-6,9-14, [16][17][18][19][20]22,[24][25][26][27][28]30,31,33,[35][36][37][38][39][40] We report a case of this PTA variant associated with aneurysm that was successfully treated with the Guglielmi detachable coils (GDCs).…”
