Case of complete mesh migration into the stomach after mesh hiatoplasty for a hiatal hernia

Abstract: Mesh migration is a rare complication of surgery for a hiatal hernia. Here, we present the case of a 72-year-old who complained of dysphasia and bodyweight loss. Upper gastrointestinal endoscopy revealed incarcerated mesh in the lumen of the esophagogastric junction. Surgery was performed under both endoscopy and laparoscopy, and the mesh was successfully removed via gastrostomy. To the best of our knowledge, our case is the first in which mesh that had migrated into the esophagogastric junction was removed by… Show more

“…The first reported case of mesh graft migration was by Herrera et al, in 1976, which migrated into the large bowel 2 years after the repair followed by the second case reported by Majeski et al, in 1998, that migrated and caused intestinal obstruction 30 years after the repair [1]. Still, the incidence of mesh graft complications has been reported up to 20%, where meshes migration being part of these complications and was recorded at an incident rate of 0.07% [10].…”

“…Likewise, inflammation related to the mesh graft secures its repair due to fibrosis, compared to loose mesh without fibrosis leading to recurrence of the hernia. Therefore, Multiple measures to reduce the rate of inevitable mesh graft repair complications are considered during the repair [9, 10].…”

“…Moreover, none of these studies described any cases of complete oesophageal obstruction caused by mesh migration that resulted in dysphagia [1][2][3][4][5]. Furthermore, the pathomechanism underlying this type of complication remains unexplained [1][2][3][4][5]. However, we are unable to clarify why patients with oesophageal perforation caused by mesh migration are generally in good condition and show no symptoms other than dysphagia [1][2][3][4][5].…”

“…Laparoscopic repair with synthetic mesh implantation is a standard procedure in the surgical treatment of symptomatic hiatal hernia [1,2]. After reviewing the current literature, we found only a few studies that described complete mesh migration into the lumen of the oesophagus that resulted as a postoperative complication of hiatal hernia [1][2][3][4][5]. Moreover, none of these studies described any cases of complete oesophageal obstruction caused by mesh migration that resulted in dysphagia [1][2][3][4][5].…”
