Osteopetrosis encompasses a spectrum of conditions marked by heightened bone density due to faulty osteoclast-mediated bone resorption, leading to an accumulation of immature bone and thickened cortical structures. This condition gives rise to bone fragility, blood cell irregularities, nerve entrapment and growth challenges, all stemming from disrupted bone remodelling. Craniofacial distinctiveness, encompassing anomalies in the skull and jaw, is a frequent occurrence. Osteopetrosis presents a range of clinical signs, including facial and dental anomalies. The diagnostic process involves thorough clinical and radiological assessments, often obviating the need for genetic testing. Interestingly, few prior reports have delved into the specifics of craniofacial and dental issues in osteopetrosis. The presented case showcases rare occurrence of maxillary osteomyelitis. The diagnosis was established through a combination of history, clinical, radiographic and laboratory findings. The patient declined surgical intervention, leading to the implementation of conservative management involving regular irrigation alongside systemic antibiotic therapy.

Osteopetrosis is a hereditary disease characterized by increased bone density due to a defect in osteoclast function and malformed bone resorption. 1 Osteopetrosis was originally described in 1904 by the German radiologist Albers-Schönberg. 2 Three types of osteopetrosis have been classified as the malign autosomal recessive type, the intermediate autosomal recessive, and the benign autosomal dominant type. 3 There are crucial clinical and radiological findings in the diagnosis of the osteopetrosis. 1 The clinical findings are commonly skeletal abnormalities such as fragile bone and osteosclerosis and hematologic diseases such as anemia or thrombocytopenia.In addition, vision defect and hearing impairment may occur in the patients. The radiological findings are generally diffuse osteosclerosis, increased cortical bone thickness, and decreased medullary canal diameter. 3 A "marble bone" appearance is usually common in the skeleton, a "bone-in-bone" appearance in the bones of the spine or the hand phalanges, and a "rugger-jersey spine" appearance in the vertebral endplates. 1 Orofacial examination findings of patients with the osteopetrosis are generally unerupted tooth, malformed crowns and roots, enamel hypoplasia, increased dental caries, and tooth loss. 3,4 Furthermore,

Objetivo: Caracterizar os casos de osteomielite, bem como identificar os fatores associados à doença ocorrida em um hospital de referência traumatológica em Teresina-PI no período de fevereiro a setembro de 2021. Métodos: Estudo observacional analítico do tipo caso-controle pareado, com a seleção de dois controles para cada caso de osteomielite. A pesquisa foi realizada com dados secundários provenientes do sistema de gestão assistencial do Hospital de Urgência de Teresina-PI. As análises foram realizadas de forma descritiva e analítica, através de modelagem estatística logística multivariada. Resultado: Houve um predomínio de pacientes mais velhos entre os casos de osteomielite.As comorbidades mais frequentes foram hipertensão, tabagismo e diabetes, que prevaleceram entre os casos. O principal motivo do trauma foram ocorrências relacionadas a acidentes de trânsito. Os casos demoraram mais dias entre o trauma e a internação/intervenção, bem como permaneceram cerca de 3x mais tempo internados na instituiçãoA bactéria isolada com maior frequência foi a Klebsielapneumoniae. A cada dia entre o trauma e a internação, o risco de osteomielite aumentou duas vezes e a cada reabordagemaumentou três vezes.O uso de prótese contribuiu com um aumento no risco em 2,26 vezes. Conclusão: A infecção óssea permanece sendo um grande problema de saúde, contribuindo para o insucesso da cirurgia ortopédica. Fatores de risco associado ao aumento da osteomielite devem sofrer intervenções modificadoras. Medidas como controle glicêmico, desencorajamento do tabagismo, tratamento de outros focos de infecção e um ato cirúrgico cuidadoso e programado, precisam ser adotados.
