Case Report: Persistent Pulmonary Hypertension of the Newborn and Narrowing of the Ductus Arteriosus After Topical Use of Non-Steroidal Anti-Inflammatory During Pregnancy

Abstract: Background: The use of non-steroidal anti-inflammatory drugs (NSAIDs) during the third trimester of pregnancy can cause premature constriction of the ductus arteriosus. This report describes a case of in utero narrowing of the ductus arteriosus (DA) diagnosed postnatally in a baby with Persistent Pulmonary Hypertension of the Newborn (PPHN), after maternal use of Diclofenac-Epolamine 140 mg patch during the second and third trimester.Case Presentation: A fetal ultrasounds revealed an enlarged hypertrophic righ… Show more

“…Diclofenac is the representative of the class that shows the greatest power to inhibit prostaglandin E2 (PGE2) production both in vitro and in vivo. Diclofenac is a low molecular weight molecule that easily crosses membranes, with an average maternal/fetus drug ratio < 1, concluding that over time it can accumulate in the fetal tissue [96,97]. Metamizole sodium, withdrawn from the US but still available in Europe and Latin America, has been studied for its toxic effect on the fetus.…”

“…However, we did not observe a dramatic change in NSAID exposure during pregnancy after this date. Regarding topical use, the complications after maternal exposure are suspected based on case reports describing use of topical diclofenac [ 41 , 42 ]. Our results were in accordance with another French cohort study, describing 60 per 10,000 exposed pregnancies after the sixth month [ 17 ].…”

“…Mefenamic acid consider as a class of a nonsteroidal anti-inflammatory medicine (NSAID), and owing to the powerful analgesic and anti-inflammatory qualities it has, it has garnered a large amount of interest in recent years. As a consequence of this, it is used extensively in the therapy of a wide variety of unpleasant diseases, including menstrual cramps, rheumatoid arthritis, and osteoarthritis, amongst others (1) . It contains numbing, anti-pyretic, and anti-inflammatory characteristics, at higher concentrations, this molecule reduces inflammation (2) .…”
