Case Report: Prolonged CSF PCR Positivity in a Neonate With GBS Meningitis

Abstract: Bacterial meningitis is one of the critical diseases that needs to be diagnosed and treated promptly. Recent diagnostics of high sensitivity and specificity rates, such as PCR, helped with such presentation, especially in cases with prior antibiotics that led to culture negativity. However, the time window of PCR positivity is not well-studied, with scattered reports of different periods of positivity. Here, we report a case of neonatal GBS meningitis with positive PCR for more than 80 days from starting antib… Show more

“…Adults previously treated with antibiotics as short as one day duration may have negative GBS culture from CSF [21] . Prolonged GBS CSF PCR positivity has been demonstrated to be related to persistent infection in a neonate with ventriculitis and residual brain abscess [22] suggesting that test positivity can be associated with residual infection. An Irish multicenter diagnostic accuracy study comparing CSF PCR with culture in infants with late-onset GBS meningitis found the test to be 100% sensitive with a specificity of 97% [23] .…”

“… 4 Gram-negative bacteria such as Klebsiella and E. coli have been reported as highly prevalent in Saudi Arabia. 5 While Haemophilus influenzae type b (Hib) (57%), Streptococcus pneumoniae (31%), and group B streptococcus (7.5%) have been identified as significant contributors to NM in Saudi Arabia. 6 A sporadic case of Elizabethkingia meningoseptica meningitis in an extremely premature infant with low birth weight was reported in Saudi Arabia.…”

“…Clinicians must be able to detect the clinical manifestations of meningitis, as well as comprehend its therapy and prevention. The elimination of the invading organism from fluid surrounding the brain and spinal cord is fully reliant on antimicrobial treatment that must be started as momentarily as conceivable following the diagnosis of bacterial meningitis [4][5][6][7][8][9] .…”
