Case Series of Gastric Hepatoid Adenocarcinoma

Abstract

Hepatoid adenocarcinoma of the stomach (HAS) is a rare form of gastric cancer that histologically resembles hepatocellular carcinoma and is characterized by large amounts of alpha fetoprotein in the serum. The prognosis of HAS is poor compared to that of primary gastric cancer with five-year survival rates of 9% and 44%, respectively. Here, we report five patients diagnosed with HAS. Our experience suggests that an advanced stage of HAS has an extremely poor prognosis, but early detection and radical surgery can help improve the prognosis of the disease. 복부 컴퓨터 단층촬영(CT)에서 위 전정부에 불규칙한 변연 을 갖는 7.4 cm의 종괴가 관찰되었고, 간(S7)에 1 cm 가량의 저음영 결절이 관찰되었다 (Fig. 1). 상부위장관내시경 검사에서 소만을 따라 위각에서 전정부에 걸친 궤양-침윤형 병변에 대해 조직검사를 시행하였다 (Fig. 2). 위내시경을 통한 겸자조직검사 에서 중등도 분화를 보이는 관상구조와 함께 부분적으로 기둥 모양의 간세포암종성 구조를 보였으며 (Fig. 3A), 면역조직화학 염색에서 AFP 양성 (Fig. 4A), hepatocyte specific antigen (HepPar-1) 음성소견을 보여 최종적으로 간세포양 위샘암종으 로 진단하였다. 간 병변 확인 및 병기 설정 위해 추가적으로 간
