Background: Malaria remains the most common specific cause of fever in returned travellers and can be life-threatening. We examined demographic and travel correlates of malaria among Canadian travellers and immigrants to identify groups for targeted pretravel intervention.Methods: Descriptive data on ill returned Canadian travellers and immigrants presenting to a CanTravNet site between 2004 and 2014 with a diagnosis of malaria were analyzed. Data were collected using the GeoSentinel data platform. This network comprises 63 specialized travel and tropical medicine clinics, including 7 Canadian sites (Vancouver, Calgary, Toronto, Ottawa, Winnipeg and Montréal), that contribute anonymous, delinked, clinician-and questionnaire-based travel surveillance data on all ill travellers examined to a centralized Structure Query Language database. Results:During the study period, 20 345 travellers and immigrants were evaluated, and 93% had a travel-related diagnosis. Of these, 437 (2.1%) patients received 456 malaria diagnoses, the most common species being Plasmodium falciparum (n = 282, 61.8%). People travelling to visit friends and relatives were most well-represented (n = 169, 38.7%), followed by business travellers (n = 71, 16.2%). Sub-Saharan Africa was the most common source region, accounting for 341 (74.8%) malaria diagnoses, followed by South Central Asia (n = 55, 12%). Nigeria was the most well-represented source country, accounting for 41 cases (9.0%). India, a high-volume destination for Canadians, accounted for 40 cases (8.8%), 36 of which were caused by Plasmodium vivax. Of 456 malaria diagnoses, 26 (5.7%) were severe. Of 377 nonimmigrant travellers with malaria, 19.9% (n = 75) travelled for less than 2 weeks, and 7.2% (n = 27) travelled for less than 1 week. Interpretation:This analysis provides an epidemiologic framework for Canadian practitioners encountering prospective and returned travellers. It confirms the importance of preventive measures and surveillance associated with travel to sub-Saharan Africa and India, particularly by travellers visiting friends or relatives. Abstract. Zika virus (ZIKV) is a mosquito-borne flavivirus with a significant public health impact highlighted by the ongoing epidemic in the Americas. We describe a 44-year-old male presenting to our tropical medicine center with complaints of fever, headache, joint pain, and rash after recent travel to Guyana. The patient subsequently developed gait imbalance and lower extremity weakness with clinical examination, cerebrospinal fluid studies, and magnetic resonance imaging of the spine consistent with a diagnosis of Guillain-Barré syndrome (GBS). ZIKV infection was confirmed via detection of ZIKV RNA in urine by polymerase chain reaction. The patient was treated with intravenous immunoglobulin and experienced near-complete neurologic recovery, reporting ongoing mild paresthesia up to 2 months later. This case highlights the diagnostic challenges posed by ZIKV and underscores the need for clinician awareness of the potential for neurological complications such as GBS with ZIKV infection. Child Travellers CountThe World Tourism Organization estimates that in 2013 tourism will continue growing and that in 2020 there will be 1.6 billion international trips taken. In 2011, there were 13.3 million trips taken abroad in Spain. There are very few figures for the number of international trips taken by Spanish children, but it is known that they form about 10% of travellers, a similar percentage to that found in US studies. Furthermore, during the last decade the immigrant population of Spain has grown, increasing the number of families returning to their home country to visit friends and relations. Although children make up a significant proportion of international travellers there is little information on pre-travel medicine for this group. The aim of this article is to describe the particularities of general advice, vaccination recommendations and malaria prevention for child international travellers, with the purpose of both infectious and non-infectious disease prevention.La Organización Mundial del Turismo estima que en el año 2013 el turismo continuará creciendo y que en el año 2020 los viajes internacionales van a ser 1,6 billones. En 2011 se realizaron en España 13,3 millones de viajes al extranjero. Las estimaciones de niños españoles que realizan un viaje internacional son muy escasas, pero se sabe que estos representan aproximadamente un 10% de los viajeros, porcentaje parecido al mencionado en estudios realizados en EE. UU. Además, en la última década en España ha aumentado la población inmigrante, lo que incrementa el número de familias que retornan a sus países de origen para visitar a sus amigos y familiares. Aunque los niños representan una parte importante de los viajeros internacionales, existe poca información acerca de la medicina previaje en este grupo de población. El objetivo de este artículo es describir las particularidades, en el niño que realiza un viaje internacional, de los consejos generales y las recomendaciones de vacunación y prevención de la malaria; se pretende prevenir no solo las enfermedades infecciosas, sino también las no infecciosas.
