Cataract Surgery with a Refractive Corneal Inlay in Place

Abstract: Purpose. To present a case of cataract surgery performed in a patient with a refractive corneal inlay in place. Methods. A 48-year-old female patient presented to our institute with bilateral cataract. The patient had undergone refractive corneal inlay implantation three years ago in her right, nondominant eye for presbyopia correction. Biometry and intraocular lens (IOL) power calculation were performed without removing the inlay. Phacoemulsification and IOL insertion were carried out in both eyes in a usual … Show more

“…There are several case reports describing the opposite sequence of events, where patients with prior corneal inlay implantation subsequently undergo cataract extraction with intraocular lens placement, either with or without initially removing the inlay. These patients had previously received any one of the three major corneal inlay types, including the Kamra inlay [ 19 ], Presbia Microlens inlay [ 20 ], and the Raindrop Near Vision Inlay [ 21 ]. These anecdotes suggest that cataract surgery can still be successfully performed after inlay implantation, which will be important for those young presbyopes opting for surgical treatment of their presbyopia.…”

“…При этом происходит образование плотной капсулы и, как следствие, снижение прозрачности роговицы. Актуальным представляется поиск биосовместимого материала, инертного по отношению к ткани стромы роговицы, в присутствии которого КСР обладают сниженной тенденцией к адгезии и пролиферации [7][8][9][10]. Среди доступных эластичных материалов, имеющих положительный опыт применения в офтальмохирургии (ИОЛ, интрастромальные роговичные сегменты, внутрикапсульные кольца и др.…”

“…75 In clinical practice, movement of accommodating IOLs has been shown to be insufficient to result in large changes in the power of the optical system. 76 At the moment, there is no perfect presbyopia-correcting IOL currently available to meet the needs of all patients.…”
