We have conducted a systematic review of the scientific literature concerning outcome and clinical effectiveness of prostheses used for primary total hip replacement (THR) in Norway. The study is based on two Health Technology Assessment reports from the UK (Faulkner et al. 1998, Fitzpatrick et al. 1998), reviewing the literature from 1980 to 1995. Using a similar search strategy, we have evaluated the literature from 1996 through 2000. We included 129 scientific and medical publications which were assessed according to a specific appraisal protocol. The majority (72%) were observational studies, whereas only 9% were randomized studies. We could not retrieve any peer-reviewed documentation for one third of the implants. The Charnley prosthesis had by far the best and most comprehensive evidence base with better than 90% implant survival after about 10 years. Survival of the Charnley prosthesis declines by about 10% during each of the two following decades. Except for the Charnley and Lubinus IP, no other prosthesis on the market in Norway has given long-term results (> 15 years). 5 other cemented implants have given comparable results at about 10 years of follow-up. Some uncemented stems have shown promising medium-term outcome, but no combination of uncemented cup and stem fulfilled the benchmark criterion of > or = 90% implant survival at 10 years, which we propose as a minimum requirement for unrestricted clinical use for prostheses used in primary THR. New or undocumented implants should be introduced through a four-step model including preclinical testing, small series evaluated by radiosterometry, randomized clinical trial involving comparison with a well-documented prosthesis, and finally, surveillance of clinical use through registers.

Background:There are concerns with regard to the femoral fixation in cementless total hip arthroplasty in elderly patients. We report a retrospective analysis of clinical and radiological results of uncemented metaphyseal fit modular stem in elderly patients irrespective of anatomic characterstics of proximal femur.Materials and Methods:This study reviews the outcomes of 60 primary hip replacements using a metaphyseal fit modular stem (third-generation Omniflex stem) conducted in 54 patients, of age 75 years or older. After a mean follow-up of 10,4 years, complete clinical and radiographic records were available for 52 hips of 48 patients. The patients were evaluated by Harris Hip Score (HHS).Results:There was a significantly improved pain score and Harris Hip Score (41,6 to 83,2). Six stems (11.53%) were revised: four because of periprosthetic fracture; one stem was well fixed, but presented a large osteolytic lesion in the metaphyseal area and the last stem was revised because of aseptic loosening. Stem survival taking aseptic loosening as the end-point was 98%. Bone atrophy in the proximal femur caused by stress shielding was observed in 39 stems (75%), but there was no case of subtrochanteric stress shielding. Moreover, atrophy appeared within two years postoperatively, with no extension thereafter.Conclusions:We achieved good clinical and radiographic results by uncemented metaphyseal fit femoral stem regardless of patient's age and femoral canal type.

Resumo Objetivo Avaliar os resultados clínicos e radiográficos da haste femoral não cimentada Logical (Baumer, Mogi Mirim, SP, Brasil) nas artroplastias totais do quadril (ATQs). Método Foi realizada uma coorte retrospectiva de 632 pacientes submetidos à artroplastia primária do quadril por coxartrose nos quais foram implantadas hastes femorais não cimentadas Logical. Todos os pacientes eram provenientes do ambulatório de Cirurgia do Quadril da instituição. O período do estudo compreendeu de janeiro de 2004 a janeiro de 2015. O desfecho definido para avaliar a sobrevida da haste foi a indicação clínica e radiográfica da revisão ou a revisão propriamente dita da haste femoral por qualquer causa. Resultado Observaram-se curvas de sobrevida estimadas > 95%, com um seguimento variando de 2 até 13 anos. Houve baixa incidência de fraturas periprotéticas transoperatórias (0,02%). Não foi observada migração axial ou atrofia óssea cortical na amostra radiográfica avaliada. Conclusão Os resultados demonstram, no médio prazo, excelente sobrevida da haste femoral não cimentada Logical. Embora ainda sejam aguardados estudos no longo prazo, este implante parece ser seguro e promissor para ser utilizado nas artroplastias primárias do quadril.
