Causes of death in patients with rheumatoid arthritis autopsied during a 40-year period

Abstract: We studied causes of death (CoDs) between 1952 and 1991 assessed by a clinician before autopsy and then determined at autopsy by a pathologist in 369 subjects with rheumatoid arthritis (RA) and 370 subjects without RA (non-RA). We analysed clinical data for RA subjects between 1973 and 1991. In RA subjects, leading autopsy-based CoDs were RA, cardiovascular diseases and infections. Between diagnoses of CoDs by the clinician and those determined by the pathologist, RA subjects had lower agreement than did the n… Show more

“…Recently, results from an English RA inception cohort reported that the mortality of patients with RA increased within the first 7 years of the disease, and coronary heart disease was mainly responsible for this reduced survival 91. On the other hand, an autopsy study indicated that coronary deaths are diagnosed less accurately in patients with RA than in non-RA subjects 92. A more frequent use of analgesics among patients with RA may suppress the ischaemic symptoms and hamper the clinical diagnosis of coronary heart disease.…”

“…Interestingly, available pathomorphological evidence suggests that occlusive atherosclerotic coronary pathology rarely contribute to cardiovascular mortality in RA [5,6], and it is likely that systemic rheumatoid inXammation predisposes to a variety of coronary manifestations (e.g., coronaritis, vasospasm, and microvascular disease), which may destabilize atherosclerotic coronary disease in stress-associated conditions. Investigation of these conditions and their potential association with cardiovascular morbidity and mortality in RA merits further investigation.…”

“…Gastrointestinal (GI) disease is one of the most common comorbidities that can adversely affect the quality of life and the lifespan of patients with rheumatoid arthritis (RA) 1,2,3,4,5,6,7 . There is extensive research showing increased risk of upper GI disease, particularly, drug-related upper GI disease, in RA 8,9,10,11,12,13 .…”
