Cavernous Sinus Dural Arteriovenous Fistula Associated With the Development of Sphenoidal Ridge Meningioma -Case Report-

Abstract: A 76-year-old man presented with a cavernous sinus (CS) dural arteriovenous fistula (AVF) associated with the development of a meningioma without venous sinus occlusion. Initial digital subtraction angiography did not reveal the CS dural AVF, which appeared simultaneously with the enlargement of the meningioma and lead to right oculomotor nerve paresis. In this case, the development of meningioma possibly increased the vascular tumor bed and affected the venous hemodynamic return, thus leading to the dural AVF. Show more

“…[11] Meningeal tumor is often reported as associated with dAVF. [1][2][3][5][6][7][8][9][13][14][15][16][17] Most previous reports describe single cases, but a series of ten cases was reported in 2014. [2] However, the tumor and dAVF were located far apart in all cases, with no cases that occurred at the same site.…”

“…Only ten cases of dAVF in the vicinity of meningeal tumor invading the venous sinuses are shown [Table 1]. [2,3,[5][6][7][8][9]13,15,16] Five of these ten cases occurred in the transverse-sigmoid sinus, [2,3,5,6,16] four cases were tumor invasion of the SSS with concomitant dAVF, [8,9,13,15] one of which was not resected and was treated only as dAVF, [15] and two were hemangiopericytomas. [8,9] Only one case was meningioma.…”

“…1,5,7,13,14,17,34,40 DAVFs have also been reported in the setting of postoperative sinus manipulation, head trauma, and less frequently in the setting of meningiomas. 4,8,12,18,30 Several reports have identified hypercoagulable states in patients with DAVFs, including prothrombotic genes, or acquired hypercoagulopathy associated with malignancy, pregnancy, or infection. 23,25,30,36 However, the prevalence of such states could not be gleaned from case reports, and reported series often included inconsistent case selection, and variable and often limited hypercoagulability surveys.…”

“…18 In some cases, DAVFs may occur secondary to other abnormalities as well, such as after the endovascular treatment of carotid-cavernous fistulas, 19,20 neoplastic venous sinus thrombosis, 21 or increased venous pressure associated with meningioma growth. 22 Inflammatory diseases affecting the dura, such as idiopathic hypertrophic pachymeningitis, viral and bacterial meningitis, were described to cause DAVFs to the straight, cavernous and superior sagittal sinuses respectively. 23-25 Blunt 26-31 or penetrating 32 traumatic brain injury may also be causative factors.…”
