CD3+ lymphocytosis in the peri-implant membrane of 222 loosened joint endoprostheses depends on the tribological pairing

Abstract: Background and purpose — The most frequent cause of arthroplasty failure is aseptic loosening—often induced by particles. Abrasion material triggers inflammatory reactions with lymphocytic infiltration and the formation of synovial-like interface membranes (SLIM) in the bone–implant interface. We analyzed CD3 quantities in SLIM depending on articulating materials and possible influences of proven material allergies on CD3 quantities.Patients and methods — 222 SLIM probes were obtained from revision surgeries o… Show more

“…The evaluation of the tissue response pattern(s) is also performed in addition to the identification of implant wear and non-implant related particles for a comprehensive pathological report. The correlation between type of particulate wear and inflammatory response can be important for the choice of the most host-compatible materials and tribological couplings [ 80 ]. Due to rapidly advancing developments in prosthesis materials and prosthesis design, new types of wear particles and associated inflammatory response patterns can be detected in the periprosthetic tissue and SLIM by histological examination.…”

“…M1 polarization of macrophages was observed in association with implant-induced inflammation and necrosis [71][72][73]. There is growing evidence for a role of lymphocytes in the mechanism of AL and PPOL, particularly in relation to hypersensitivity [74][75][76]. Neutrophils are also important in the reaction to wear particles and to microbial loads [77,78].…”

“…Higher accumulation of lymphocytes was already previously reported in the tissues around MoM THA (ref. 35 ), where the levels of metals are significantly much higher than in case of MoP or CoP bearing pairs. In fact, higher frequency of T lymphocytes is not only a domain of MoM pairs, it was also observed in the tissues around MoP total joint arthroplasties 36,37 .…”
