Cefadroxil versus penicillin in the treatment of streptococcal tonsillopharyngitis

Abstract: In a randomized multicentre study the efficacy of phenoxymethyl-penicillin and cefadroxil was tested in the treatment of acute tonsillopharyngitis caused by group A streptococci. These organisms were detected in 269 of the 300 children studied. Of the 239 patients in whom results could be evaluated, 121 received penicillin and 118 cefadroxil. After ten days of therapy the microbiological failure rate was 19% in the penicillin group and 6.8% in the cefadroxil group (p less than 0.01). Ten of 23 cases of microbi… Show more

“…There was also a clear difference in the choice of antibiotics for the treatment of recurrent tonsillitis. Several investigations have shown a decrease in the relapse rate if an oral cephalosporin or clindamycin is used instead of penicillin V for the treatment of pharyngeal tonsillitis (25,26). Based on those studies, clindamycin or a cephalosporin is recommended for the treatment of recurrent tonsillitis in Sweden.…”

“…In the present study, three doses of cefcanel daloxate were chosen to bracket the doses of cephalosporins that had been used in other tonsillitis studies (4)(5)(6)(7)(8)(9). The prodrug characteristics, pharmacokinetics and betalactamase stability of cefcanel daloxate were expected to produce better tolerance and less bacteriological failure .…”

“…Cephalosporins have been shown to be more effective than penicillin V in treating primary beta‐hemolytic group A streptococcal pharyngotonsillitis (37, 55, 67). Cephalosporins may enable shorter treatment regimens than penicillin V, and some patients may only need to be dosed once daily (68).…”
