Cell Kinetics of Hepatic Metastases as aPrognostic Marker in Patients WithAdvanced Colorectal Carcinoma

Abstract: Cell kinetics was determined, as 3H-thymidine labeling index (LI), in hepatic lesions from 36 patients with primary colorectal carcinoma; LI values ranged from 0.9% to 23.5% and were normally distributed. Cell kinetics was not related to sex or age of the patient, or to liver function. For clinical studies the median LI value of 10% was used to separate slowly and rapidly proliferating lesions. Univariate analysis showed that patients radically resected and with a low LI tumor have a longer disease-free interv… Show more

“…The traditional parameters selected for the analysis are known to play an independent role in the prognosis of patients with CLM (Tranberg & Bengmark 1994). Our report confirms and reinforces a previous prospective study on the prognostic role of cell kinetics, evaluated as [3H]thymidine labelling index, on both freedom from progression and overall survival in CLM (Silvestrini et al 1990). Measurement of cell proliferation might provide prognostic information and a rational basis to develop more specific clinical treatments for the group of patients with resectable CLM.…”

“…The first is that simultaneous surgical management of the primary and metastatic site is an approach with considerable potential benefits. The second is that synchronous CLM comprise a special subgroup of CLM, with particular biology and prognosis, and require specifically tailored studies to evaluate the various therapeutic strategies. Several strategies have been devised over the past 20 years to combine the various treatment modalities required for the management of synchronous CLM.…”

“…Moreover, 3H-TdR LI was an even more important additional significant discriminant of survival within patients with resectable liver metastases. In fact, for this subgroup, all patients with low 3H-TdR LI (<lo%) metastases and only about one-third of patients with high 3H-TdR LI metastases had a probability to be alive at 12 months [27]. MONTHS CONCLUSIONS Histologic grading and clinico-pathologic staging of colorectal carcinomas are still considered the best indicators of prognosis and guides to treatment.…”
