Central nervous system melioidosis in the pediatric age group: review

Abstract: Pediatric neurologic melioidosis is very rare. Meningoencephalitis is the most common presentation. Mortality is the highest in neonates. Ceftazidime appears to be the drug of choice in intensive phase, although the best drug in maintenance phase cannot be commented upon, especially in very young children.

“…Only 1.5 to 5 percent of the melioidosis cases have been reported to have neurological involvement [1,2]. Although there is a systematic review conducted in pediatric patients [3], most CNS melioidosis cases occur in adults. Therefore, the knowledge of the CNS melioidosis including epidemiology, clinical manifestations, laboratory findings, treatment, and outcome are still limited and solely depend on data of case reports.…”

“…The adoptive transfer of B. pseudomallei -loaded CD11b cells via the tail vein resulted in abscesses in the brain that were predominately localized in the regions surrounding the cerebral superior sagittal sinus 10 . In children with melioidosis who develop meningoencephalitis, brain abscesses are more often localized in the parietal lobe than in other regions 2 . In this study, the endotoxemic mice often exhibited abscesses localized in the dural venous sinus of the parietal lobe.…”

“…Melioidosis is a fatally infectious disease caused by Burkholderia pseudomallei infection that usually occurs in endemic areas, such as northern Australia and Southeast Asia. The clinical spectrum of melioidosis varies and can present as localized cutaneous suppuration, multiple organ abscesses and neurological disorders 1 , 2 . The severity of melioidosis depends on the invasive route, the size of inoculation, the virulent characteristics of the bacterium and host defense factors 1 , 3 5 .…”

“…Pediatric neuro melioidosis is very rare and meningoencephalitis is the most common presentation. Cranial nerve palsies and fever are prominent presenting features,[ 7 ] the commonest cranial nerve affected being the facial nerve. [ 8 ] Our patient initially presented as unilateral facial palsy along with low-grade fever.…”
