Cervical Lymph Node Metastasis in High-Grade Transformation of Head and Neck Adenoid Cystic Carcinoma: A Collective International Review

Abstract: Adenoid cystic carcinoma (AdCC) is among the most common malignant tumors of the salivary glands. It is characterized by a prolonged clinical course, with frequent local recurrences, late onset of metastases and fatal outcome. High-grade transformation (HGT) is an uncommon phenomenon among salivary carcinomas and is associated with increased tumor aggressiveness. In AdCC with high-grade transformation (AdCC–HGT), the clinical course deviates from the natural history of AdCC. It tends to be accelerated, with a … Show more

“…High grade transformation (or dedifferentiation), defined as severe nuclear atypia, brisk mitotic activity, prominent necrosis, and loss of the biphasic ductal-myoepithelial differentiation, has been reported in ACC (28, 36, 37). Although, distinguishing solid ACC from ACC with HGT might be sometimes challenging, the aggressive histologic features of solid ACC in general appear to be extensive and exaggerated in ACC with HGT compared to solid ACC.…”

“…Adenoid cystic carcinoma is a malignancy of the salivary glands with a mean survival of 4.3 years and 43–57% of patients present with lymph node metastasis 1. d-ACC is a rare variant with high-grade transformation and is reported in only 40 case reports worldwide 1 2…”

“…Neck lymph node metastasis is the most common way of tumor metastasis in HNSCC. Approximately 50% patients with HNSCC have occult or obvious cervical lymph node involvement at diagnosis . Although surgical treatment, radiation therapy, and chemotherapy have improved significantly, the overall survival rate of the people with the disease has not improved significantly over the past 20 years.…”
