Letter to the EditorThe validity of claim-based cesarean deliveries in Japan Dear Editor, Using the Japan National Database (NDB) of Health Insurance Claims for 2014, we estimated the cesarean delivery rate to be 18.6% for all pregnancies and 82.7% for women with multiple pregnancies. 1 The NDB data for the number of overall and prefectural cesarean cases in September 2014 1 closely matched that of the Medical Institution Survey 2 (intraclass correlation coefficient = 0.996, p < 0.001). However, there were no official data for cesarean deliveries for multiple pregnancies.To evaluate the validity of claim-based cesarean deliveries, including multiple pregnancies, we investigated deliveries in Akita Prefecture. In this prefecture, clinics with fewer than 20 beds do not provide obstetric care for women with multiple pregnancies, and most (78%) deliveries occur at hospitals with 20 or more beds. 2 Therefore, we asked hospitals with 20 or more beds to report the total number of cesarean and transvaginal deliveries performed in 2014, including deliveries involving multiple pregnancies. All 16 hospitals responded to our survey, reporting a total of 809 cesarean deliveries, 28 of which involved multiple pregnancies. These numbers were similar to the respective 733 and 31 estimated by the NDB (Table 1). Cesarean delivery rates at the hospitals were as low as 15.8% overall and 44.4% for multiple pregnancies,
