Challenges and controversies in the conservative management of uterine and ovarian cancer

Abstract: Uterine cancer is the fifth most common cancer in women worldwide with an estimated 320,000 annual diagnoses. Its most common form, endometrioid adenocarcinoma of the endometrium (endometrial adenocarcinoma [EAC]), is thought to develop through excessive proliferation of endometrial glands, and then increasing steadily in incidence. “…However, obesity negatively impacts pregnancy rates and lowers the probability of long-term treatment success among women receiving fertility-sparing treatment [ 85 , 86 ]. This remains an ongoing challenge to conservative management of EC, and high-quality data from randomized controlled trials are needed to successfully address this issue [ 87 ].…”

“…In 2019, approximately 61,880 new cases of UC and 12,160 deaths from the disease were estimated [75], with the number of UC cases expected to increase in older populations. UC includes endometrial adenocarcinoma (EA) (the most common type), adenosquamous carcinoma, papillary serous carcinoma, and uterine sarcoma [76,77]. Patients in stage I disease have a good prognosis and can be cured with surgery therapy.…”

“…Adenexal masses suspected to be cancerous should undergo immediate staging laparotomy as they constitute only 10%-15% of all ovarian cancer cases [11] . Primarily resected ovarian cancer mandates adjuvant therapy as per the guidelines, except for grade 1 or 2 stage I disease [11] .…”

Primarily resected ovarian cancer mandates adjuvant therapy as per the guidelines, except for grade 1 or 2 stage I disease [11] . In the current situation of the COVID-19 pandemic, single-agent carboplatin can be considered a suitable option as there is limited evidence of benefit of combination therapy in early stage ovarian cancer.…”
