In 2015, a dust explosion occurred in New Taipei Water Park in Taiwan, and 499 casualties was appallingly high. In fact, the government spent 208 min of response time from receiving calls for emergency to sending the last wounded person to hospital. To stop a similar accident happening again, a well-prepared strategy, with an aim to reduce the rescue time, to respond to such an emergency is necessary. This study mainly focuses on the execution of mass casualty operations while both efficiency and quality of care are considered. We not only find that there is only 1 h or less to allocate rescue resources after a Burn Mass Casualty Incident (BMCI) happens but also shorten the response time. It is our first time to use a special method, called Critical Path Method (CPM), to analyze the rescue process. But before CPM is created, the benefit of available resources should be maximized, and timely, safe, and effective emergency medical services should be provided. With CPM, the interrelations of every activity can be visualized, and the most time-consuming activity can be found by this efficient time management. If the relationship between the predecessors and its successors operations of the most time-consuming activity item is reshaped, BMCI prehospital scheduling management becomes more effective based on its efficiency and quality of care.

Background: The goal of this study was to examine the effect of hospital emergency department (ED) regionalization policy and the categorization of hospital emergency care policy (categorization policy) on patient appropriate ED use.Methods: We conducted an observational study of the effect of emergency care policy intervention on patients' visits. Seven years of data from the Taiwan National Health Insurance Research Database (NHIRD) were examined. Taiwan implemented a nationwide three-tiered hospital ED regionalization and categorization policies in 2007 and 2009, respectively. There were 1,835,860 ED visits found among one million random samples from the NHIRD between 2005 and 2011. ED visits were categorized by the modified New York University ED algorithm. A time series analysis was performed to examine the change in the rate of appropriate ED use after the policy took effect.Results: Total ED visits increased by 10.7% from 2005 to 2011. Average appropriate ED visit rate was 66.9% during the policy intervention. The trend in the appropriate ED visit rate showed no significant policy effect.Conclusions: Provider side of regionalization and categorization policies did increase emergency care accessibility. However, regionalization and categorization policies no significant effect on patient appropriate ED use.

Background and AimsThe effective response of emergency medical services in mass casualty incidents (MCIs) calls for sufficient preparation. The components of preparation must be determined first to achieve this goal. This study aimed to describe the elements of preparedness of emergency medical services for MCIs.MethodsA qualitative study was carried out on emergency medical service systems in Iran (from April 2022 to mid‐March 2023), using in‐depth semistructured interviews with participants who were managers and members of the incident command team, experts, technicians, paramedics, and telecommunicators of emergency medical services. Interviews were carried out face‐to‐face and via telephone. The data were collected using voice recorder and transcript and analyzed by content analysis method. This study was conducted using the consolidated criteria for reporting qualitative research.ResultsThirty‐six participants were included in the study. A total of 834 codes were analyzed. Thirteen components were extracted from the study and classified as five categories including “Strengthening management and organization,” “individual and group empowerment,” “capacity expansion,” “technology and infrastructure development,” and “operational response measures.”ConclusionEmergency medical service preparedness in response to MCIs is a critical issue. For improving preparedness, the main components must be identified. The study results described the elements of emergency medical service preparedness, which could be used as a framework for developing the national model of emergency medical service preparedness in MCIs.
