Challenges of diagnosing and managing bronchiectasis in resource-limited settings: a case study

Abstract: Bronchiectasis, once an orphan disease is now gaining renewed attention as a significant cause of morbidity and mortality. It is a morphologic term used to describe abnormal, irreversibly dilated and thick-walled bronchi, with many etiologies. The management of bronchiectasis can be challenging because its pathogenetic mechanisms is still evolving. Its diagnosis and management is particularly more demanding especially in resource-limited settings like Nigeria because of delayed diagnosis and improper managemen… Show more

“… [7] It is known that most LMICs experience resource limitations, so there is a financial barrier to extensive investigations. [16 , 18] A second limitation is that this was a single-centre study, so not all the findings can be extrapolated to other institutions and/or regions in SA. Thirdly, this was a retrospective record review, and details for some of the information were missing in the patient files.…”

“…HRCT is considered expensive in Nigeria, largely owing to poverty and lack of health insurance. [18] Additionally, relatively few medical centres in SA are equipped with computed tomography scanners. It therefore appears likely that there may be financial reasons for the low rate of HRCT imaging.…”

“…Bronchiectasis is a progressive respiratory disease characterized by chronic airway inflammation resulting in irreversibly dilated, thick-walled bronchi and progressive decline in lung function [ 19 ]. Criteria for surgery included unsuccessful medical therapy with recurrent respiratory infections, persistent sputum production, chronic cough, persistent lung abscess or hemoptysis, and localized bronchiectasis [ 12 ].…”

“…Bronchiectasis is a progressive respiratory disease characterized by chronic air way inflammation resulting in irreversibly dilated, thick walled bronchi and progressive decline in lung function [ 1 ]. It commonly affects proximal and medium sized bronchi [ 1 ]. Although it is regarded as an orphan disease in the developed world, it is still major cause of respiratory morbidity for children of developing nations as most are undiagnosed and untreated leading to early decline of respiratory function [ 2 ].…”
