Changes in endolymphatic hydrops in patients with Ménière’s disease treated conservatively for more than 1 year

Abstract: In the three ears in which the symptoms alleviated, EH was reduced in two ears, but EH was reduced in only one of 17 ears in which the symptoms did not alleviate. The Fisher exact test revealed that EH reduction occurred more frequently in ears with alleviation of the symptoms (p < 0.05).

“…Differences may also exist between the times of delayed postcontrast imaging, most likely being performed after 24 hours, but in some cases after 4 hours 28 . The impact of this discrepancy on the imaging evaluation of MD patients is unclear: Suga et al were able to assess the presence of vEH in 4 of 6 patients with definite MD independently from the time of acquisition 28 .…”

The presence of EH in other audiological disorders can be shown by IT administration: Hornibrook et al assessed the presence of EH not only in patients with definite MD (14/30 ears), but also in patients with other audiological disorders (3/45 ears), even if at a very low rate (6%) 29 .…”

“…Suga et al [14] examined 20 ears using MRI, EH improved in two of three ears in which also showed improved symptoms with 1 year of conservative treatment, however, EH improved in only one of 17 ears that showed no symptom improvement. A relationship between reduction in EH and improvement in symptoms has been noted [14].…”

A relationship between reduction in EH and improvement in symptoms has been noted [14]. If MD were simply due to increased endolymphatic pressure, the more severe the MD, the greater would be the hearing ability threshold with increasing G width; further, with improvement in MD, G width would decrease and the hearing ability threshold would also decrease.…”

“… 11 , 16 18 However, subjective grading using only three categories (i.e., significant, mild, and none) might not be sensitive enough to monitor subtle treatment effects. 10 , 11 , 14 , 19 Quantification of EHs has been the goal of several studies. 3 , 11 , 20 A highly sophisticated method has been proposed using high contrast data obtained by IT-GBCM.…”
