Purpose To determine the correlation between the preoperative basal diameter of macular hole, the postoperative area of high autofluorescence (AF) in macula, and visual acuity in full-thickness macular hole. Methods Forty-nine patients with full-thickness macular hole who underwent vitrectomy and C 3 F 8 filling were reviewed. The preoperative diameter of macular hole, the 6 months postoperative area of high AF in macula if it existed, the length of inner segment/outer segment (IS/OS) defect, and visual acuity were obtained. The correlation between them was determined. Results At postoperative 6 months, the rate of high AF in macula was 63.3%. There were statistical differences between with and without high AF groups in postoperative best-corrected visual acuity (BCVA) (t = -2.751, p = 0.008), preoperative basal diameter of macular hole (t = -4.946, p = 0.00001), and postoperative length of IS/OS defect (t = -8.351, p<0.00001). Simple linear regression analysis showed high positive correlations between preoperative basal diameter of macular hole and area of high AF (p<0.00001, r = 0.893), postoperative length of IS/OS defect and area of high fundus AF (FAF) (p<0.00001, r = 0.779), and negative correlations between area of high AF and postoperative BCVA (p = 0.037, r = 0.375). There was low correlation between diameter of macular hole and postoperative BCVA (p = 0.112). Conclusions The preoperative basal diameter of macular hole and postoperative length of IS/OS defect decides the postoperative area of high AF in macula to some degree, and the postoperative area of high AF in macula can be an evaluating indicator for poor macular function recovery.

Идиопатическое макулярное отверстие (ИМО) -одна из наиболее частых причин снижения централь-ного зрения в возрастной группе старше 65 лет, стра-дают преимущественно женщины (67% всех случаев) [1]. Возникновение ИМО обусловлено прогрессиру-ющей витреоретинальной тракцией в фовеальной зо-не [2]. Вероятность развития макулярного отверстия прямо пропорциональна возрасту, поэтому рост про-должительности жизни людей в развитых странах де-лает проблему диагностики и лечения ИМО все более актуальной. Кроме того, наблюдения показали, что при наличии ИМО на одном глазу риск развития ана-логичного процесса на парном глазу составляет 7-12% в течение последующих 5 лет и 17% -в тече-ние 10 лет [3]. Самопроизвольное закрытие сквозно-го макулярного дефекта в результате спонтанной зад- Цель -исследовать характер и диапазон нарушений электрофизиологических показателей сетчатки при идиопатическом макулярном отверстии (ИМО), проанализировать наличие и степень их взаимосвязи со структурными параметрами сетчатки. материал и методы. Обследованы 249 человек (280 глаз) с ИМО. всем пациентам проводили регистрацию ганцфельд-электроретинограммы (гЭрГ) и мультифокальной электроретинограммы (мфЭрГ), определение электрической чувствительности сетчатки, лабильности зрительного анализатора и критической частоты слияния мельканий, статическую компьютерную периметрию, оптическую когерентную томографию сетчатки. Параметры глаз с ИМО сравнивали с аналогичными показателями контрольной группы (25 человек , 49 глаз) и 196 парных глаз без ИМО. Результаты. Анализ параметров мфЭрГ показал, что наличие ИМО во всех случаях приводит к выраженным изменениям биоэлектрического потенциала центральной зоны сетчатки. в 62% случаев снижение плотности и удлинение латентности P1 захватывает, помимо фовеа, пара-и перифовеа, в 8% случаев -всю центральную зону сетчатки в пределах 30°. в 30% случаев отмечено перемещение зоны максимального биоэлектрического потенциала из центра в область пара-и перифовеа, где зарегистрированы значения его амплитуды и плотности, супернормальные для этих отделов сетчатки (феномен сдвига). выявлена слабая достоверная корреляция электрофизиологических показателей и значений толщины сетчатки вокруг разрыва. Заключение. Наиболее информативный метод исследования нарушения функций сетчатки при ИМО -мфЭрГ: в 100% случаев регистрируется резкое снижение плотности и нарушение конфигурации биопотенциала сетчатки в фовеальной зоне, в 70% случаев эти нарушения распространяются за пределы фовеа.
