Changes in refractive error in patients with accommodative esotropia after being weaned from hyperopic correction

Abstract: When emmetropisation occurs early in patients with AET, it is necessary to note the development of myopia and the deterioration of ocular alignment.

“…There has always been a concern regarding striking a balance between achieving bifoveal fixation and avoiding interference with emmetropization by prescribing full hypermetropic refractive correction when treating accommodative ET in young children. The management of accommodative ET includes (1) identifying risk factors for the development of accommodative ET, (2) providing optimal refractive correction without interfering with the process of emmetropization, and (3) weaning of glasses when possible[ 9 10 11 12 ] The current study aims to describe the clinical characteristics and outcome in Omani children with accommodative ET. To our knowledge, no similar study has been conducted in Oman.…”

“…The results showed that most eyes were in a mild overcorrection state in the earlier postoperative stage (about 1 week) but the overcorrection subsided at 1 month and stabilized thereafter. This may have occurred because the hyperopic astigmatic eye may have over-accommodated for a long period [ 21 , 22 ]. Although this surgery corrected the refractive error, the over-accommodation continued during the early stage after surgery and patients need more time to adapt to the new refractive state; and as time passed, the accommodation relaxed and overcorrection disappeared gradually [ 8 ].…”

“…This differs from the pattern in the general hyperopia population. [5,7,8] Furthermore, the incidence of hyperopia in patients with esotropia is larger than that in the normal population. RAE is associated with hyperopia, [9,10] and the refractive error generally ranges from 2 to 6 diopters (D).…”
