Changes in Right Ventricular Function Measured by Cardiac Magnetic Resonance Imaging in Patients Receiving Pulmonary Arterial Hypertension–Targeted Therapy

Abstract: P ulmonary hypertension (PH) is a hemodynamic and pathophysiological state defined as increased mean pulmonary artery pressure ≥25 mm Hg at rest (assessed by right heart catheterization [RHC]).1 PH can be precapillary (pulmonary wedge pressure ≤15 mm Hg, normal or reduced cardiac output) where the changes occur in the pulmonary arterial circulation or postcapillary (pulmonary wedge pressure >15 mm Hg) where the left ventricle (LV) or the mitral valve is likely to be involved. Clinical Perspective on p 114In pr… Show more

“…47 Cardiac MRI-derived parameters, such as right ventricular and left ventricular volumes and ratios, mass, and ejection fraction correlate with traditional measures of functional status, including 6MWD, 46,48,49 hemodynamics, 50 and survival. [50][51][52][53][54] In addition, viewing MRI parameters in the context of other standard risk parameters may allow us a better spatial assessment of risk of the continuum of each individual factor ( Figure 6).…”

“…74 The prognostic value of changes in RV mass in PAH is small and seems limited to patients with scleroderma PAH. 69,75,76 The limited clinical value of the assessment of RV mass compared with RV volumes is perhaps explained by the fact that RV hypertrophy both reflects the severity of pulmonary vascular remodeling (negative impact) and is part of the normal adaptive process to an increased load (positive response). 77 For example, despite a similar elevation in PAP, patients with Eisenmenger syndrome have a greater amount of RV hypertrophy compared with patients with idiopathic PAH, which is associated with better RV function and survival.…”
