Objective To determine recent insurance claim rates for, facility income from, and the roles of professionals associated with management of nutrition, oral ingestion and oral health maintenance in long‐term care insurance facilities (LTCIFs) in Niigata Prefecture. Methods A questionnaire on current staffing, oral health professional (OHP) utilisation, and claims for insured benefits for the investigated services was mailed to all LTCIFs in Niigata Prefecture, Japan (n = 304). Claim rates for and average facility income from these benefits were calculated. Facility income was compared between facilities with and without employed OHPs. Statistically significant factors associated with claims for investigated benefits were identified by logistic regression. Results Responses from 111 facilities indicated that they made insurance claims for nutrition management (95%), transition from tube feeding to oral ingestion (9%), basic maintenance of oral ingestion (39%), additional services for maintenance of oral ingestion (23%), oral health management system (68%), and oral health management (17%). Most facilities established collaborations with private dental clinics, but only 16% of facilities employed OHPs. Facility income was significantly higher (P = 0.005) for facilities that employed OHPs. OHP employment by facilities was associated with claims for four of the six benefits (P < 0.05). Conclusions Most facilities consulted with private dental clinics, and 16% of the facilities employed dentists or dental hygienists to help residents manage oral ingestion problems and oral health maintenance. The facility income associated with management of these problems was significantly higher in facilities employing dental professionals.

Osteoporosis is an insidious and increasingly prevalent disease that can cause fractures and affect patients' quality of life. The current study comparatively evaluates patients with and without loss of bone mass in terms of salivary calcium, viscosity, and pH. A controlled cross-sectional study was conducted in two groups of 32 postmenopausal women subjected to a bone densitometry scan and later referred for dental management at the Federal University of Minas Gerais, Brazil. The patients were assigned to two groups: Group 1 - patients with low bone mineral density (BMD) and Group 2 - patients without bone mineral changes. The following salivary parameters were evaluated: calcium concentration, flow rate, viscosity, pH, and average total protein. An oral examination was performed for assessment of DMFT variables and tongue coating. Data were analyzed using descriptive and inferential statistics, adopting a p-value < 0.05. The patients' mean age was 60 years (± 7.35). Salivary flow, pH, and viscosity were similar among the groups. Average total protein was 14.8 mg/mL and 19.0 mg/mL in Groups 1 and 2, respectively. Tongue coating and salivary calcium levels were significantly higher in Group 1 (p < 0.001). Salivary calcium is an important screening tool and may eventually be used for the diagnosis of bone mineral changes.

The effects of the oral exercises on the oral function of people with dementia

The objective of this study was to examine the effects of the home-based oral exercises on the oral function in people with mild to moderate dementia. The patients with mild to moderate dementia at King Chulalongkorn Memorial Hospital, who were randomly allocated to�an experimental group (n = 11), were asked to perform 3 home-based oral exercises for 3 months while those in a control group (n = 11) did not do any oral exercises. The oral function including maximum tongue pressure (MTP), oral diadochokinesis (ODK) rates of /ta/, /ka/, /pa/, swallowing function (modified water swallowing test: MWST), and oral moisture were evaluated at baseline and at the end of the 1st, 2nd, and 3rd months. In addition, the nutritional status including the mid-upper arm circumference (MUAC), calf circumference (CC), body mass index (BMI), and the Mini Nutritional Assessment-Short Form were evaluated as consequences of changed oral function at baseline and after 3 months. The data of 19 participants (10 in the experimental group and 9 in the control group), who were�completely evaluated at the�end-point visit, were analyzed. The results showed that the MTP and ODK rates of /ta/, /ka/, and /pa/ in the experimental group improved significantly after the 3-month program, with notable interaction effects between time and intervention. Nevertheless, there was no significant interaction effect�on the oral moisture, and the MWST scores in two groups were unchanged. Furthermore, after 3 months, the MUAC, CC, and BMI�in the experimental group increased significantly, with notable interaction effects between time and groups. The total exercise compliance rate was 96.5%. The compliance was�significantly negatively correlated with age. There was no reported serious adverse event.�In conclusion, the home-based oral exercises performed in this study could improve the tongue strength and motor function of tongue and lips in people with mild to moderate dementia. The caregivers, who were trained to control the�exercises, were crucial to the success of the oral exercises.
