Changes in thyroid hormone state in children receiving chemotherapy

Abstract: These results demonstrate that, in children, thyroid hormone state changes significantly during chemotherapy, apparently not related to physical well-being but to the drugs administered. Future investigations should focus on the impact for patient care and possibilities of (preventive) intervention.

“…It is known that several drugs influence thyroid function and metabolism 7,26 . This has recently been demonstrated in paediatric cancer patients during chemotherapy 27 . In our study, we were able to show an independent correlation between thyroid abnormalities in patients without irradiation to the head/neck regions and trofosfamide and idarubicin treatment, although there were only a few patients receiving these drugs.…”

“…The criteria employed for l ‐thyroxine (T4) supplementation treatment were unclear in our cohort. Only 53% (19/36) of patients with overt and subclinical hypothyroidism received l ‐T4 supplementation, even though it is recommended in affected children 17,27,31 . Data on thyroid cancer as a second malignancy were not available.…”

“…We include this description of these subjects to avoid the lost to follow-up because of sudden death [22] and the diagnostic bias due to other exposure status. Specifically, patients with known autoimmune diseases [23] or using drugs that may interfere with thyroid function [24,25] are prone to develop autoimmune thyroiditis or subclinical hypothyroidism. Furthermore, 2 subjects refused to continue the study and 3 were lost to follow-up.…”

“…Although it is well known that radiation exposure of the thyroid gland can induce hypothyroidism, its potential influence can be ignored in this study because radiation-induced hypothyroidism would be detected several years after completion of irradiation 48. Furthermore, possible influence of CDDP-based chemotherapy on thyroid function has been reported in children 49.…”
