Changing Oral Contraceptives from Prescription to Over‐the‐Counter Status: An Opinion Statement of the Women's Health Practice and Research Network of the American College of Clinical Pharmacy

Abstract: Addressing the issue of unintended pregnancy is a national priority. One proposed strategy to reduce unintended pregnancy is to improve access to oral contraceptives by changing them to over-the-counter (OTC) status. Existing data indicate that oral contraceptives meet safety criteria required of OTC products. Available literature demonstrates that women can self-screen for contraindications to oral contraceptives and can do this as well as clinicians, and experience with OTC emergency contraception suggests t… Show more

“…Furthermore, CPs may assume that patients who request a specific medicine are already knowledgeable about its use, and that asking questions and giving additional advice would be patronizing and could lead to a negative patient reaction [57,58]. Additionally, while U.S. pharmacy graduate programs have curricular requirements encompassing the various aspects of hormonal contraceptive service provision, including screening, prescribing, and monitoring [20], Brazilian pharmacy graduate programs are deficient in this respect.…”

“…Pharmacy-based screening for safe use of COCs, and counseling regarding the correct use, benefits, and adverse effects of COCs can improve patient adherence and safety [19]. Based on the results of previous studies indicating the potential and ability of CPs to provide effective clinical services in the community setting, the American College of Clinical Pharmacy (ACCP) supports the change of COCs to OTC with the condition that they are sold where a pharmacist is on duty [20]. The ACCP also states that there is a need to evaluate how COCs are actually dispensed, in order to identify the CPs’ role in COC access [20].…”

“…Although the relative safety of this method has been well-established and some have advocated for making OCs available over-the-counter (OTC) [2,3], women interested in starting or continuing with this method must still obtain a prescription from a health care provider in the US. One of the justifications for maintaining the prescription-only status is that, during their clinician visit, women are screened for health conditions such as hypertension and diabetes, which may increase their risk of myocardial infarction, stroke or other complications while taking OCs [4].…”

“…In 2011 an opinion statement by the Women’s Health Practice and Research Network of the American College of Clinical Pharmacy highlighted the importance of changing the oral contraceptive pill to an over-the-counter product. 27 This statement emphasized the importance of making the contraceptive pill more accessible to help reduce the rate of unintended pregnancies. Before the oral contraceptive pill is made available over-the-counter is it vital that pharmacists are trained adequately on the various aspects of this service.…”
