Introduction: Mental health and neurological disorders are prevalent in Pakistan. However, there are considerable concerns with their management due to issues of access, availability of trained personnel and stigma alongside paucity of such data. Consequently, there is a need to document current treatment approaches starting with tertiary hospitals in Pakistan where patients with more severe mental and neurological disorders are typically treated. Subsequently, use the findings to help direct future policies and initiatives. Methods: Multi-centred, cross-sectional, prospective study principally evaluating current medicine usage among patients attending tertiary hospitals in Pakistan with psychiatric and neurological disorders. In addition, possible factors contributing to the prevalence of these disorders in this population to help with future care. All 23 tertiary care hospitals in the ten major Districts in Pakistan were included, which cover 75% of the population. Results: 57664 patients were evaluated of which 35.3% were females. Both females and males had multiple brain disorders and multiple co-morbidities. Schizophrenia was the most prevalent disorder overall among both females (25.2%) and males (30.4%). A median of six medicines were prescribed per patient, with antipsychotics and antidepressants the most prescribed medicines. Clozapine was the most prescribed medicine in males (12.25%) and females (11.83%) including for psychiatric disorders, with sodium valproate the most prescribed medicine in epilepsy in males (42.44% of all anti-epileptic medicines) as well as females (46.38%). There was a greater prevalence of both disorders among the lower classes. A greater prevalence of schizophrenia was seen in patients abusing alcohol and smokers. The divorce rate was higher among the studied patients and the prevalence of depression was higher among the widowed population. Conclusions: There were concerns with the quality of prescribing including the extent of polypharmacy as well as possible overuse of clozapine especially in patients with epilepsy, both of which need addressing.

Background: Pakistan is confronted with significant mental health challenges, which necessitate the urgent need for a comprehensive assessment of its mental healthcare system. A holistic understanding of the mental health landscape is essential to identify strengths, weaknesses, and existing gaps within the system, which can inform targeted interventions and policy enhancements to improve mental healthcare delivery and accessibility. Objective: The primary objective of this study is to conduct an in-depth analysis of Pakistan's mental healthcare system across various dimensions, guided by the World Health Organization's Assessment Tool for Mental Health Systems (WHO-AIMS). Methods: Data for this study wereobtained through desk research, including an examination of existing policies and legislation, and consultations with various health facilities across Pakistan. This comprehensive analysis focused on six domains within the WHO-AIMS framework: (1) policy and legislation, (2) mental health services, (3) integration of mental health into primary care, (4) public awareness and collaboration with other sectors, (5) human resources, and (6) monitoring and research initiatives. Results: The findings from our analysis revealed valuable insights within the six domains. We identified areas of strength and opportunities for improvement in Pakistan's mental healthcare system. These insights serve as the foundation for revising and updating national priorities for mental healthcare delivery and advocacy efforts. Key areas of focus include enhancing policy and legislation, expanding access to mental health services, improving existing initiatives for better integration of mental health into primary care, improving public awareness and public-private partnership, addressing human resource challenges, and strengthening monitoring and research initiatives. Conclusion: This study provides a roadmap for refining and enhancing Pakistan's mental health ecosystem and informs the prioritization of mental health campaigning efforts. Ultimately, our study contributes to ongoing efforts to improve mental healthcare accessibility and quality in Pakistan.

Berdasarkan penelititan (Malik, 2019) tentang Pola Peresepan Psikotropika Asia Timur (Research on East Asia Psychotropic Prescription Pattern) pada tahun 2017, ditemukan adanya tren peresepan polifarmasi antipsikotik yang dikombinasikan dengan anxiolitk, antidepresan dan antiparkinson. Pada penelitian (Arozal, 2019) di Indonesia, ditemukan adanya pola pengobatan antipsikotik yang kurang baik pada pasien skizofrenia yaitu pengobatan kombinasi antipsikotik yang memiliki risiko tinggi terhadap kejadian interaksi obat yang relevan secara klinis dan lebih dari 40% pemberian antipsikotik tanpa indikasi yang sesuai serta lebih dari 50 % pasien menerima dua atau lebih antipsikotik secara bersamaan.…”

Pada pasien skizofrenia, penyakit fisik atau komorbid merupakan salah satu penyebab yang potensial untuk mengurangi harapan hidup penderitanya. Penggunaan psikofarmaka bersamaan dengan obat untuk terapi komorbid memiliki risiko terhadap kejadian masalah terkait obat (Drug Related Problems). Identifikasi kasus masalah terkait obat(DRP) pada penelitian ini dilakukan terhadap obat yang digunakan oleh pasien yang dirawat di ruang stabilisisasi RSJ dr. H Marzoeki Mahdi Bogor selama periode Agustus – Oktober tahun 2021. Metode penelitian menggunakan disain cohortdengan pendekatan prospektif dan data dianalisis secara deskriptif. Klasifikai kejadian DRP dilakukan sesuai dengan klasifikasi PCNE versi 9.0 yang ditetapkan oleh Pharmaceutical Care Network Europe Association. Dari 125 pasien skizofrenia yang diamati, diperoleh data demografi pasien skizofrenia disertai komorbid adalah sebagian besar dengan jenis kelamin pria sebanyak 74 pasien (59.2 %), 59 pasien (47,2 %) tergolong kelompok usia 41 – 60 tahun, dan jenis penyakit penyerta tertinggi yang ditemukan adalah hipertensi berjumlah 77 pasien (61.6 %). Kasus DRP yang telah diidentifikasikan sebanyak 56 kasus DRP pada 44 pasien, dengan kasus terbanyak pada domain P2.1 yaitu efek obat yang merugikan (yang dapat terjadi) sebanyak 32 kasus (57,14%). Penyebab terjadinya DRP berkaitan dengan pemilihan obat (C1) yaitu C1.3 : Tidak ada indikasi untuk obat 15 kasus (26,79%), C1.4 : Kombinasi yang tidak tepat 7 kasus (12,50%), dan C1.7: banyak obat untuk satu indikasi 10 kasus (17,85%). Jumlah obat yang digunakan pasien, dan komorbiditas berpengaruh terhadap kasus DRP yang ditemukan pada pasien (p value< 0,05).
