Changing trends in HPV vaccination in Japan

Abstract: After the Japanese Ministry of Health, Labor and Welfare (MHLW) suspended its recommendation for the human papillomavirus (HPV) vaccine in June 2013, the rate of members of the new target population receiving of at least one of three doses of HPV vaccine declined, reaching 0.3% in 2016. Recently, however, the monthly number of vaccines delivered to healthcare facilities has significantly increased, from 878 doses over December 2016-April 2017 to 35,396 doses over January-March 2021. This may be due to governme… Show more

“…Under this system, reports could be made by not only physicians who can perform medical evaluation, but also by third parties such as vaccinated individuals, their guardians and others, without proper medical evaluation of the duration of time since vaccination. 3 Adverse event reports, including those from past vaccinations, rapidly increased within a short period, making it necessary to temporarily suspend the active vaccination recommendation, according to the precautionary principle. However, research findings indicate that this suspension has been excessively long and may have harmed many females.…”

“…2 This may be due to governmental efforts to convey information about the benefits of HPV vaccination to the eligible population and their caregivers, as well as local educational programs. 3 Females aged 16–24 years, who had missed their opportunity to receive the free HPV vaccination under the national immunization program when they were 11–16 years old, due to the recommendation suspension, will now have the opportunity to receive the missed vaccinations over the last three years, free of cost. Furthermore, females who have received these vaccines at their own expenses, may apply for reimbursement.…”

“…But, based on the reported adverse events (pain and motor dysfunctions), the proactive vaccination was suspended in June 2013. Soon, the vaccine coverage dropped to <1%, despite its availability in the program [ 15 ]. With no association of these symptoms with the vaccine/vaccination documented and targeted communication efforts, the coverage started rising 2018–19 onwards and in November 2021, the Japan government decided to resume the proactive HPV vaccination [ 16 ].…”

“…As a result, the vaccination rate of the target population has been estimated to be below 1% [ 5 , 6 ]. The incidence of HPV vaccination has been changing in recent years, with quadruple the number of HPV vaccination doses supplied in the first 3 months of 2021 compared to the same period 4 years ago [ 7 ]. Although the trend in the HPV vaccination rate seems to have been increasing in Japan, it is still important to find better ways to promote HPV vaccination—not only in Japan but also in other countries that have not accomplished the 90% vaccination goal.…”
