IntroductionThe prevalence of dementia is increasing without a known cure, resulting in an increasing number of informal caregivers. Caring for a person with dementia results in increased stress and depressive symptoms. There are several behavioural interventions designed to alleviate stress and depressive symptoms in caregivers of persons with dementia with evidence of efficacy. Two of the best-known interventions are the New York University Caregiver Intervention (NYUCI) and the Resources for Enhancing Alzheimer's Caregivers Health (REACH). The effectiveness of the NYUCI and REACH has never been compared. There is also a paucity of data on which interventions are more effective in Hispanics in New York City. Thus, we proposed the Northern Manhattan Hispanic Caregiver intervention Effectiveness Study (NHiCE), a pragmatic clinical trial designed to compare the effectiveness of adaptations of the NYUCI and the REACH in informal Hispanic caregivers of persons with dementia in New York City.Methods and analysisNHiCE is a 6-month randomised controlled trial comparing the effectiveness of adaptations of the NYUCI and REACH among 200 Hispanic informal adult caregivers of persons with dementia. The planned number of sessions of the NYUCI and REACH are similar. The primary outcome measures are changes from baseline to 6 months in the Zarit Caregiver Burden Scale and Geriatric Depression Scale. Our primary approach to analyses will be intent-to-treat. The primary analyses will use mixed random effects models, and a full information maximum likelihood approach, with sensitivity analyses using generalised estimating equation.Ethics and disseminationNHiCE is approved by the Institutional Review Board of Columbia University Medical Center (protocol AAAM5150). A Data Safety Monitoring Board monitors the progress of the study. Dissemination will include reports of the characteristics of the study participants, as well as a report of the results of the clinical trial.Trial registration numberNCT02092987, Pre-results.

Caregiver burden and stress in psychiatric hospital admission

Objective: to evaluate the relation between sociodemographics factors, stress and burden of care of family caregivers of patients at a psychiatric hospital admission. Method: quantitative study, with a cross-sectional correlation design. A total of 112 family caregivers participated, older than 18, in a Brazilian psychiatric hospital. A sociodemographic questionnaire was used to collect data, the Zarit Burden Interview and LIPP Adult Stress Symptom Inventory. Results: burden of care in family caregivers at a psychiatric hospital admission was significantly associated with stress (p=0.000). The psychological symptoms of stress predicted severe burden. Most caregivers presented a moderate or severe burden, with 52.7% in the resistance phase of stress; 66.1% presented psychological symptoms. Conclusion: results show the alarming situation of caregivers of patients from a psychiatric hospital, evidencing their own vulnerability to illness. Indeed, the during admission in a psychiatric hospital, not only patients need care, but also their caregivers.

Kronik ruhsal hastalıklar, bireylerin duygu, düşünce ve bilişsel yetilerinde bozulmalara, kişilik ve bireysel alışkanlıklarında değişimlere, sosyal ve ekonomik kayıplara yol açmaktadır. [1,2] Ruh sağlığı hizmetlerinin sunulmasıyla ilgili çağdaş modeller toplum temelli tedavilere ve uzun süreli hastane yatışlarının önlenmesine önem vermektedir. [3,4] Kronik ruhsal hastalığı olan bireylerin %10'u uzun vadede bakıma ihtiyaç duymakta ve çoğu aileleri ile birlikte yaşamaktadır. [4,5] Günümüzde hastayı toplumda ve kendi ortamı içinde tedavi etme yaklaşımının benimsenmesiyle, hasta yakınları birincil bakım verici konuma gelmiş ve sorumlulukları artmıştır. Bu durum beraberinde bir dizi sorunu getirmiştir. [6-8] Ailesiyle yaşayan hastaların çoğu tüm gününü evde geçirmekte, aileden en az bir kişi hasta bakımı nedeni ile sosyal yaşam ve üretim sürecinden uzak kalmakta ve aileler fiziksel, ruhsal, duygusal, sosyal ve ekonomik sorunlar yaşamaktadır. [4,9-11] SİSTEMATİK DERLEME / SYSTEMATIC REVIEW SUMMARY Objectives: This systematic review analyzes caregiver burden in chronic mental illness and the factors associated with it. Methods: The study was conducted by checking the Turkish Psychiatry Index, the Turkish Medical Index, the Turkish Medline, the National Council of Higher Education Thesis Center and the PubMed databases. The database research was performed in English and Turkish using keywords and synonyms for them. Twenty-three studies were included in this study. Results: The majority of the studies included in the review were descriptive and studied schizophrenic patients' caregivers. They used a variety of measurement instruments. It was found that the burden perception of caregivers was moderate to severe and was influenced by different variables. Conclusion: Experimental studies are insufficient to determine the burden of caregivers in Turkey, so experimental and qualitative studies should be conducted with different specific samples.
