Background: Preterm birth, delivery prior to 37 completed weeks or 259 days gestation, is a worldwide maternal and perinatal challenge and is a leading cause of neonatal morbidity and mortality. Preterm birth remains the leading cause of perinatal and postnatal mortality and morbidity especially in developing countries where the health care services are suffering from limited resources. Premature babies usually suffer from both immediate and long term consequences. Right after birth, they have difficulties in breathing, temperature regulation, bleeding, infection and other problems due to organ immaturity. Their growth and developmental milestones will also be affected leading poor physical, mental, educational and psychosocial problems as a long term consequences. Preterm deliveries were responsible for 1 million out of the 6.3 million deaths of children under 5 in 2013 [1]. In Kenyatta National hospital, few studies have been carried out to determine the prevalence and factors associated with preterm birth. Hence the aim of this study is to determine the prevalence and factors associated with preterm birth at Kenyatta national hospital (KNH), Nairobi, Kenya. Materials and Methods: This was a hospital based descriptive cross-sectional study involving randomly selected respondents (N = 183) from post natal ward of Kenyatta National Hospital. Systematic random sampling method was applied to recruit the study respondents. A pre-tested semi-structured questionnaire was employed to collect information on the possible determinants of Preterm birth. Data was analysed using SPSS software version 22.0. Descriptive analysis was done using mean and frequency proportion. Inferential analysis using chi-square test was used to establish association different variables. The ethical approval to conduct the study was obtained from KNH-University of Nairobi Ethical Review Committee (KNH-UoN ERC). Permission to collect data was sought from the KNH and consent was obtained from the selected respondents before admin- The determinants of preterm birth are multifactorial including history of abortion, preterm birth, urinary tract infection, hypertension and alcohol consumption during pregnancy. Most of these risk factors of preterm birth are controllable if reproductive age mothers are educated properly. It is very important for antenatal mothers to adhere to the guidelines of antenatal visits so that those at risk are spotted and close monitoring can done in order to reduce this high rate of preterm birth and its negative consequences. Strategies to avert the high prevalence of preterm birth and its associated morbidity and mortality must be given priority at national, regional and international levels, so that the Millennium Development Goal (MDG) 4 can be achieved.

Background: Preterm birth (PTB) can be caused by different factors. The factors can be classified into different categories: socio demographic, obstetric, reproductive health, medical, behavioral and nutritional related. The objective of this review was identifying determinants of PTB among mothers who gave birth in East African countries. Methods: We have searched the following electronic bibliographic databases: PubMed, Google scholar, Cochrane library, AJOL (African journal online). Cross sectional, case control and cohort study published in English were included. There was no restriction on publication period. Studies with no abstracts and or full texts, editorials, and qualitative in design were excluded. Funnel plot was used to check publication bias. I-squared statistic was used to check heterogeneity. Pooled analysis was done by using fixed and random effect model. The Joanna Briggs Critical Appraisal Tools for review and meta-analysis was used to check the study quality. Results: A total of 58 studies with 134,801 participants were used to identify determinants of PTB. On pooled analysis, PTB was associated with age < 20 years (AOR 1.76, 95% CI: 1.33-2.32), birth interval less than 24 months (AOR 2.03, 95% CI 1.57-2.62), multiple pregnancy (AOR 3.44,95% CI: 3.02-3.91), < 4 antenatal care (ANC) visits (AOR 5.52, 95%

Background: Preterm labor is a major cause of death and an important cause of congenital disabilities. According to WHO, preterm delivery is a labor that occurs between 20 weeks' gestation to less than 37 weeks, calculated from the first day of the last menstrual on a 28-day cycle. This study aims to determine the characteristics of pregnancy and preterm delivery at Sanglah Hospital during the year 2016. Methods: This is a retrospective research using descriptive methods; data were taken from Sanglah Hospital medical record. All cases of preterm labor in Sanglah Denpasar Hospital delivery room from 1 January to 31 December 2016 were used, information starting from the initial diagnosis at admission and the patient's medical record number were also recorded. Data were then processed with tabulation method in accordance with the characteristics of preterm labor cases. The results data were then analyzed descriptively using SPSS 16 software. Results & Discussions: The results showed that within 1 year from 1 January to 31 December 2016, from all deliveries, there was 195 (18.6%) preterm birth cases in midwifery Sanglah Denpasar Hospital. Preterm labor was most common at the age of 20-35 years old (77.9%), at the age of pregnancy 32 to <37 weeks (67.6%). Preterm birth occurs highest in mothers with primigravida (42.5%). This research also found that in 22 cases with preterm birth, previously had experienced preterm labor, where 19 cases (9.7%) had 1 preterm delivery, and 3 cases (1.5% ) had a history of 2 previous preterm deliveries. Based on medical history experienced by a mother with preterm delivery at Sanglah Hospital Denpasar, 49 (25.1%) had hypertension in pregnancy. Babies born preterm in Sanglah Denpasar Hospital period 1 January to 31 December 2016 most had a birth weight between 1500 to <2500 grams, where 150 babies (70%) was born with vigorous counts. There were 167 infants (78%) who were preterm born in Sanglah Denpasar Hospital from January 1 to December 31 2016 treated with joint care, and the remaining 29 (13.5%) were admitted to the Neonatal Intensive Care Unit (NICU). Conclusions: This research is a preliminary study that can be used as reference data about the characteristics of preterm labor in obstetrics and gynecology Sanglah Hospital Denpasar Periode January 1 to December 31, 2016. Given the importance of these data, this study may be used as basic data to do a continuous research, to find out all other characteristics. Also to find the occurrence of preterm labor and its complications, so in the near future can be used as a consideration for the management of preterm labor, also reduce and anticipate complications that could happen.
