Characteristics of Academic Medical Centers and Ischemic Stroke Outcomes

Abstract: Background and Purpose-Data supporting the efficacy of stroke center characteristics are limited. Methods-A questionnaire detailing stroke treatment practices was sent to 42 academic medical centers in the University Health Systems Consortium. In-hospital mortality of all emergency department admissions for ischemic stroke at these institutions was evaluated in a database of discharge abstracts during 1997-1999. Institutional characteristics were evaluated as predictors of in-hospital mortality after adjustmen… Show more

“…It is possible that the lack of significant findings associated with the Brain Attack Coalition items might be because more detailed information is required to understand the particular context and application of these factors within each hospital. 23 Although previous studies have found the Brain Attack Coalition items to be highly correlated and to have limited variability between hospitals, 11,23 we did not find this to be the case. Most of the characteristics were present in Ϸ60% to 80% of hospitals, which probably reflects the fact that the prototypes included a broader range of hospitals.…”

“…There have been several previous reports evaluating the impact of neurologists on the quality and efficiency of stroke care. 23,25,26 A study of stroke outcomes in academic medical centers found that in-hospital mortality was lower in those centers that had a vascular neurologist. 23 A study of Medicare recipients found DVT indicates deep venous thrombosis; GA, Georgia; IV-tPA, intravenous tissue plasminogen activator; MA, Massachusetts; MI, Michigan; OH, Ohio.…”

“…5 Although the American Stroke Association does not explicitly mandate that a neurologist be the sole provider of care to patients with acute stroke, best practices indicate that VNs should play the leading role in stroke units. [9][10][11] The American Board of Psychiatry and Neurology began certifying VNs in the United States in 2005 as a way to formally recognize those who completed special training and who demonstrate competence in all aspects of vascular neurology care. 12 The background of those VNs is diverse.…”

“…Categories of information analyzed included demographic characteristics, such as age, gender, race, and insurance status, as well as other clinical characteristics such as teaching status of the hospital, length of stay, admission priority, stroke center designation, location of the hospital, and whether or not the patient received tPA. All of these were categorized based on previous, similar studies [7,[10][11]. Age was categorized as follows: less than 60 years, 60-69, 70-79, 80-89, and 90 years or older.…”
