This study aimed to correlate postmortem TC, HDL, non-HDL, apo A1 and apo B levels with the presence of atherosclerotic cardiovascular disease and ethanol consumption before death. A total of 215 forensic autopsy cases (177 males and 38 females between 25-88 years of age) with a postmortem interval of 5.5-118.5 h were examined and divided into groups according to the presence and severity of atherosclerotic lesions and the cause of death. The presence of more advanced types of atherosclerotic lesions and a higher degree of coronary artery stenosis were associated with higher postmortem levels of Apo B and a higher apo B/apo A1 ratio. Deaths caused by acute and chronic ischemic heart disease differed from other causes of death by higher apo B levels. These findings occurred despite postmortem apo B levels being lower than the suggested clinical range in most cases. Alcohol consumption before death was not correlated with the postmortem levels of examined lipids and lipoproteins. Key points •Postmortem apo B levels were lower than the clinical range.

Zusammenfassung Hintergrund und Ziel der Arbeit In Deutschland vollzieht sich ein stetiger demografischer Wandel, welcher zu einer zunehmenden Alterung der Gesellschaft führt. Ziel der Arbeit war die Analyse der natürlichen Todesfälle mit einem Sterbealter ≥ 65 Jahre, da die gesundheitliche Vulnerabilität dieser Altersgruppe an Bedeutung gewinnt. Material und Methoden Retrospektiv wurden die Obduktionsgutachten aller natürlichen Todesfälle der ≥ 65-Jährigen im Institut der Rechtsmedizin des Universitätsklinikums der Goethe-Universität Frankfurt am Main in einem Zeitvergleich (Zeitraum I: 2000–2002; Zeitraum II: 2013–2015) ausgewertet. Ergebnisse In den Zeiträumen I und II wurden insgesamt 1206 Obduktionen in dieser Altersgruppe ermittelt. Davon wiesen 404 (33,5 %) eine nichtnatürliche Todesart auf, in 39 Fällen (3,2 %) lag eine Kombination aus natürlichem und nichtnatürlichem Tod vor, und in 94 Fällen (7,8 %) war die Todesart unklar. Die Mehrheit (n = 669; 55,5 %) verstarb an einer natürlichen Todesart. Die größte Gruppe davon (n = 350; 52,3 %) betraf kardiale Todesursachen, gefolgt von 132 (19,7 %) respiratorischen und 47 (7,0 %) abdominellen Todesursachen. Zudem lagen 37 (5,5 %) maligne Neoplasien, 37 (5,5 %) sonstige natürliche Todesursachen, 33 (4,9 %) Rupturen großer Gefäße und 33 (4,9 %) zerebrale Todesursachen vor. Im Vergleich der Zeiträume I und II fiel eine signifikante Abnahme der kardialen Todesursachen auf. Es kam insbesondere zu einer signifikanten Abnahme der hochgradigen bis verschließenden Koronarsklerosen. Zwischen beiden Geschlechtern zeigten sich signifikante Unterschiede. So wiesen Männer signifikant mehr Bypässe, Stents und Herznarben auf und erlitten ca. 10 Jahre vor den Frauen einen Myokardinfarkt. Diskussion/Schlussfolgerung Die Ergebnisse decken sich größtenteils mit der Literatur. Die Abnahme kardialer Todesursachen könnte auf eine zunehmend bessere medizinische Versorgung und eine signifikant zunehmende Implantationsrate von Stents zurückzuführen sein. Die Rolle der forensischen Gerontologie wird –gerade in Pandemiezeiten– zunehmend an Bedeutung gewinnen.

Background Although sexual activity can cause moderate stress, it can cause natural death in individuals with pre-existing illness. The aim of this study was to identify additional pre-existing health problems, sexual practices, and potential circumstances that may trigger fatal events. Methods This medicolegal postmortem, retrospective, and prospective study is based on data of autopsies performed at the Institute of Legal Medicine of the University hospital, Goethe-University, Frankfurt/Main, Germany. Outcomes Identification of pre-existing health problems, sexual practices, and potential circumstances than could trigger fatal events. Results From 1972 to 2016 (45 years) approximately 38,000 medicolegal autopsies were performed, of which 99 cases of natural death were connected to sexual activities (0.26%). Except for eight women, men represented most cases. The women’s mean age was 45 years (median = 45) and the men’s mean age was 57.2 years (median = 57). Causes of death were coronary heart disease (n = 28), myocardial infarction (n = 21) and reinfarction (n = 17), cerebral hemorrhage (n = 12), rupture of aortic aneurysms (n = 8), cardiomyopathy (n = 8), acute heart failure (n = 2), sudden cardiac arrest (n = 1), myocarditis (n = 1), and a combination of post myocardial infarction and cocaine intoxication (n = 1). Most cases showed increased heart weights and body mass indices. Death occurred mainly during the summer and spring and in the home of the deceased. If sexual partners were identified, 34 men died during or after sexual contact with a female prostitute, two cases at least two female prostitutes. Nine men died during or after sexual intercourse with their wife, in seven cases the sexual partner was a mistress, and in four cases the life partner. Five men died during homosexual contacts. Based on the situation 30 men were found in, death occurred during masturbation. Of the women, five died during intercourse with the life partner, two died during intercourse with a lover or friend, and in one case no information was provided. Clinical Translation Natural deaths connected with sexual activity appear to be associated with male sex and pre-existing cardiovascular disorders. Most cases recorded occurred with mistresses, prostitutes, or during masturbation. If death occurs, the spouse or life partner might need psychological support. Strength and Limitations To our knowledge, the present study contains the largest collection of postmortem data on natural deaths connected with sexual activities. However, the cases presented were of forensic interest; a larger number of undetected cases especially in the marital or stable relationship sector must be assumed. Conclusion Patients should be informed about the circumstances that could trigger the “love death.”
