Characteristics of optic disc melanocytomas presenting with visual dysfunction

Abstract: Objective and Design:A retrospective review study was designed to describe five cases of optic disc melanocytomas with tumor-related visual impairment.Participants:Five patients with optic disc melanocytoma presented with visual complaints to a tertiary eye hospital in Saudi Arabia.Materials and Methods:Demographic and clinical data were analyzed, including the results of ocular examination, lesion laterality, best-corrected Snellen visual acuity, pupillary reflex, visual field testing, color fundus photograph… Show more

“…It is well documented that even small optic disc melanocytomas can cause visual dysfunction by mechanical effects: optic disc compression causing vascular occlusions or nerve fiber layer defects [10,15]. There are also secondary effects causing visual disturbances such as tumor necrosis causing ischemic optic neuropathy [16] or CNVM development. Clinically, differentiating these tumors from melanomas can pose a challenge in spite of available guidelines to help make a clinical diagnosis [17].…”

“…Cases accompained by significant loss of visual acuity and/or visual field have also been reported. 2 Rashaed etal in their study found that tumour-related visual impairment was due to macular exudation, optic atrophy from optic nerve compression by the tumour, foveal choroidal neovascularisation, optic disc edema and peripapillary subretinal fluid. 2 So, although optic disc melanocytomas tend to be benign in behaviour, they may adversely affect visual function.…”

“…3,[7][8][9] Although melanocytomas of the optic disc tend to have a benign behaviour with slow evolution and stable vision, they may adversely affect visual function through a variety of mechanism. 2 Frequently melanocytes are identified coincidently, and visual impairment, apart from an enlarged blind spot where the tumour occupies the optic disc is rare. 2 However, some have reported severe visual loss also.…”
