Characteristics of patients with sensory neuropathy diagnosed with abnormal small nerve fibres on skin biopsy

Abstract: Clinical, laboratory and electrodiagnostic (EDX) characteristics of 62 patients with sensory neuropathy with abnormal skin biopsies were reviewed. Reduced epidermal nerve fibre density (ENFD) was seen in 71% and morphological changes with normal ENFD were seen in 29% of the patients. Patients with small fibre sensory neuropathy may have associated large fibre loss undetected by routine EDX. Identified associations included abnormal glucose metabolism, Lyme vaccination, monoclonal gammopathy, vitamin B12 defici… Show more

“…Small fiber neuropathy has many causes (Table 1) [5,14,16,27,34,[43][44][45][46][47][48] and may be the first manifestation of an underlying systemic disease. SFSN usually is length [13•, 27, 45].…”

“…The finding of a reliable and robust marker of axons in skin has yielded important insights into the biology of the peripheral nervous system in health and disease [3,4]. The technique is minimally invasive and allows quantitative analysis of unmyelinated nerve fibers of the epidermis and dermis to help establish a diagnosis of neuropathy [5] and to monitor the disorder in response to treatment [6]. The method is of greatest utility in small fiber sensory neuropathy (SFSN; Fig.…”

“…This is true to a greater degree in patients with small fiber neuropathy where 50% or more of patients have idiopathic neuropathy (Periquet et al, 1999;Venkataramana et al, 2005;De Sousa et al, 2006) (Table 1). Singer et al (2012) discussed current research being done in idiopathic neuropathy.…”

“…Dies lässt sich zum einen dadurch erklären, dass die Abnahme der Nervenfaserdichte schon früh im Verlauf einer Neuropathie nachweisbar ist und einem pathologischen Befund in der Neurographie vorausgehen kann [12]. Zum anderen kann sich eine SmallFiber-Neuropathie auch mit negativen sensiblen Symptomen präsentieren [8,22].…”
