Characteristics of puumala and Dobrava infections in Croatia*

Abstract: In this study, two different hantaviruses, Puumala virus (PUUV) and Dobrava virus (DOBV), were demonstrated for the first time to coexist and cause hemorrhagic fever with renal syndrome (HFRS) in Croatia. Phylogenetic analysis showed some differences among the nucleotide sequences of PUUV originating from Dinara mountain, which was more closely related to Austrian PUUV than other Croatian PUUV from Mala Kapela mountain. More consistency was found among the Croatian DOBV. HFRS was verified in 85 of 201 suspecte… Show more

“…Also, DOBV seropositive patients in our study had more severe renal impairment compared with the PUUV and non-id hantavirus positive patients as witnessed by the diuresis, serum urea and creatinine values during the acute stage of the illness. Our results are in line with previous reports from B&H, Croatia and Slovenia that associated DOBV with severe form of HFRS (13,14,15,16,17).…”

“…DOBV patients in our study had significantly lower diuresis values compared to PUUV which can be explained with the larger study sample of the patients included in this study. Reports from Croatia also associated DOBV infection with a more severe disease than PUUV infection, but also confirmed previous findings that even PUUV infection could lead to a severe disease (13,14,17). Markotic et al (17) observed that a significantly higher proportion of DOBV-infected patients had acute renal failure, visual disturbance, severe thrombocytopenia, and elevated levels of nonsegmented leukocytes, creatine, and total bilirubin.…”

“…During the 2002 Croatian outbreak, 401 cases of HFRS were recorded. Except for the coastal region and islands, the whole country was found to be an area endemic for HFRS [11][12][13]. In 2012, 154 patients were described, of whom 69.5% acquired the infection on the Medvednica mountain near the Croatian capital of Zagreb [14].…”

“…These include Dobrava (DOBV) and Puumala (PUUV) viruses, implicated in hemorrhagic fever with renal syndrome, and the nonpathogenic Tula (TULV) and Saaremaa viruses. [10][11][12][13][14] Rodents can also be infected with flaviviruses, orthopoxviruses, and lymphocytic choriomeningitis virus (LCMV); however, to our knowledge, there are no published reports of investigation for the presence of these viruses in Croatia. [15][16][17] Rodents in Croatia are also reservoirs of protozoan Apicomplexa parasites, including Toxoplasma and Babesia that cause toxoplasmosis and babesiosis, respectively, mainly in immunocompromised individuals.…”
