Characterization of Carbapenem-Resistant Enterobacteriaceae with High Rate of Autochthonous Transmission in the Arabian Peninsula

Abstract: To establish the role of local transmission versus possible pathogen import due to previous foreign exposure in infections caused by carbapenem non-susceptible Enterobacteriaceae in the Arabian Peninsula, 200 independent isolates collected in 16 hospitals of Saudi Arabia, Kuwait, Oman and the United Arab Emirates were studied. All strains were multidrug resistant; 42.5% of them also qualified as extremely drug resistant. The frequency of various carbapenemases varied according to the participating countries, b… Show more

“…In one study which aimed to establish the role of local transmission versus possible pathogen import due to global travel, it was determined that patients carrying bla NDM-1 -positive K. pneumoniae had no prior documented foreign exposure, hence suggesting a high rate of autochthonous infections (52). Even though the isolation frequency for E.…”

“…QMRA was performed to assess the risks associated with K. pneumoniae and A. baumannii because these two microbial agents have been reported to be common causative agents in nosocomial and non‐nosocomial infections in Saudi Arabia (Sonnevend et al, ; Uz Zaman et al, ; Yezli, Shibl, Livermore, & Memish, ). The two bacteria were isolated at different frequencies in the three types of examined produce.…”

“…Enterobacteriaceae possessing two carbapenemases have previously been observed worldwide (16) and in the region as well (6,17,18). It is of particular concern that representatives of the K. pneumoniae ST147 clone, well known for its epidemic potential (4,6), not only are present in the UAE as dual-carbapenemase-producing isolates but also develop resistance to the last-resort antibiotic, colistin, by insertion of a functional carbapenemase gene into the mgrB gene.…”

Apart from the earlier report on MS6671 (12), a similar ISEcp1-driven insertional mutation of mgrB in K. pneumoniae has been observed in a single isolate in France, but in that case, an extended-spectrum betalactamase gene (bla CTX-M-15 ) was coded on the insertion, and the strain remained susceptible to an array of antibiotics (19).…”
