Characterization of carbapenemases, ESBLs, and plasmid-mediated quinolone determinants in carbapenem-insensitive Escherichia coli and Klebsiella pneumoniae in Riyadh hospitals

Abstract: The main objective of this work was to characterize carbapenemases, extended-spectrum β-lactamases (ESBLs), and plasmid-mediated quinolone resistance (PMQR) among carbapenem-insensitive Klebsiella pneumoniae and Escherichia coli clinical isolates which were isolated from three hospitals in Riyadh. Thirty-one carbapenem-insensitive isolates (21 K. pneumoniae and 10 E. coli) were recovered from March 2014 to May 2014. Susceptibility testing and phenotypic detection tests were used to characterize the classes of … Show more

“…Previously, the rates of bla SHV containing Enterobacterales with a focus on uropathogenic isolates were reported 12.2% to 54.8% by Iranian authors. [24][25][26][27][28][29] Meanwhile, several authors have shown the global spared of SHV type β-lactamases in uro- pathogenic Enterobacterales from China, 30 India, 31 Saudi Arabia, 32 Turkey, 33 Ukraine, 34 and Morocco. 35 In our results, 4.1% of uropathogenic Enterobacterales contained bla NDM-1, which to the best of our knowledge, is the first prevalence of this gene in Guilan province (North of Iran).…”

“…Previously, Alizadeh et al, in a study conducted in the southwest of Iran (Kerman), showed that 17.2% of uropathogenic isolates were OXA-1 producers. 20 Reviewing global reports such as Brazil (60%), 38 Portugal (55%), 39 Saudi Arabia (54.8%, and 48.3%), 32,40 and Bangladesh (47.5%), 41 indicate that the occurrence of OXA-1 β-lactamases can be varied based on type and source of isolates and the prevalence of ESBLs in each region.…”

“…In a different way, bla IMP was detected in single isolate only. Reports of OXA-48 carbapenemases have been published worldwide, including Middle East and North Africa countries, for example, Saudi Arabia, Sultanate of Oman, Qatar, United Arab Emirates, Turkey, Egypt, Tunisia, Morocco, Libya and Lebanon [6,7,10,11,[20][21][22]. The results presented here provide the first insight into the coproduction of the OXA-48 like carbapenemase encoding gene and NDM-type representing MBL production in K. pneumoniae as well as the association of OXA-48 like and IMP-1 like among hospitalized cancer patients in Cairo, Egypt.…”

“…In the Middle East Northern African countries, such as Morocco, Algeria, Tunisia, Libya, Egypt, or Saudi Arabia, OXA-48-producing K. pneumoniae have been predominantly described [5,6]. On the other hand, in some regions of the Arabian Peninsula, NDM-1 was the experienced carbapenemases to the greatest extent followed by OXA-48-like carbapenemases [7].…”

“…The colistin was the most promising antibiotic with the 84% sensitivity [40]. 60% of the isolates were resistant to imipenem (IMI), and ertapenem (ERT) in a study reported from Riyadh, Saudi Arabia [41].…”
