Characterization of Isolates of Schistosoma mansoni from Egyptian Villagers that Tolerate High Doses of Praziquantel

Abstract: To determine if resistance/tolerance to the antischistosomal drug praziquantel (PZQ) is appearing in Egyptian villages within the Nile delta region, where it has been used extensively, we treated 1,607 infected villagers and observed that 321 required one additional treatment while 89 villagers required two additional treatments; 24 of the 89 were still not cured after a third dose of this drug. Eggs were isolated from fecal samples and serum was isolated from blood taken from seven villagers successfully trea… Show more

“…The doses used were 25, 50, 100, and 200 mg/kg/day. Detailed information describing results concerning the response of these isolates to PZQ can be found in our previous publication 9 . Two weeks later, the mice were killed and worms were counted from liver and mesenteric veins.…”

“…The methods used to obtain schistosome eggs from infected Egyptian villagers, to infect the snails with miracidia, and to infect mice with cercariae have been described. 9 Briefly, the eggs obtained from patients were separated from fecal debris and rinsed in saline and dechlorinated tap water. Illumination induced hatching, and snails were exposed to the resultant miracidia.…”

“…Reports of treatment failure in Senegal and Egypt [6,7], where isolates with reduced susceptibility to Praziquantel were obtained, have raised concerns about development of drug resistance. Further in vitro experiments confirmed the development of parasite resistance to Praziquantel compared with susceptible isolates [8][9][10][11].…”

“…6 % of the patients were still passing viable eggs. Several isolates were established in laboratory-maintained life cycles from eggs passed by uncured patients, and adult worms of these isolates were found to have PZQ ED 50 s 2-to 5-fold greater after PZQ treatment in mice than isolates that had been established from eggs passed before treatment by patients who had been cured (Ismail et al 1996).…”
