Characterizing the Rise of Disseminated Gonococcal Infections in California, July 2020–July 2021

Abstract: Background California has experienced an increase in reported cases of disseminated gonococcal infection (DGI). Given significant morbidity associated with DGI and the ability of Neisseria gonorrhoeae to rapidly develop antibiotic resistance, characterizing these cases can inform diagnosis, management, and prevention of DGI. Methods As part of the public health response to increased reports of DGI, we used gonorrhea surveilla… Show more

“…They may also lack the urogenital symptoms 2 that might otherwise prompt consideration of a gonorrhea diagnosis (such symptoms have been absent in the majority of CA DGI cases 1 ). Further complicating the picture, although current routine STI screening recommendations focus primarily on people younger than 25 years and men who have sex with men, 10 most CA DGI cases have instead occurred among older individuals aged 30 to 50 years (median age, 40 years 1 ), the majority of whom identified as heterosexual (and thus may not have been considered for routine gonorrhea screening). For these reasons, both disseminated and mucosal-site gonococcal infections may go initially undetected in DGI patients.…”

“…Disseminated gonococcal infections (DGI) occur when the sexually transmitted pathogen Neisseria gonorrhoeae invades the bloodstream and travels to distant sites of the body, 1 resulting in manifestations including dermatitis, migratory polyarthritis, tenosynovitis, bacteremia, septic arthritis, endocarditis, meningitis, osteomyelitis, and others. DGI is a significant public health concern, with the potential to cause serious morbidity and even mortality 2,3 .…”
