Chemoradiation therapy for cervical cancer: Toxicity of concurrent weekly cisplatin

Abstract: The incidence of severe acute hematological toxicity was significantly higher in this study than in previously reported randomized controlled trials (RCTs), especially in the group of 40 mg/m2 cisplatin. A dose of 30 mg/m2 of cisplatin was considered to be feasible in weekly cisplatin and radiation therapy.

“…Although extensive literature is available on the acute morbidity due to chemoradiotherapy in women with cancer cervix, most of the studies have been done at carefully prepared academic centers in developed countries (Rose et al, 1999;Maduro et al, 2003;Ikushima et al, 2006;Mahantshetty et al, 2010;Khalid et al, 2012). Socially disadvantaged women in less developed countries of the world may be more susceptible to the combined treatment's toxic effects due to poor nutritional status, presence of comorbid chronic conditions and of difficulties in accessing medical care during treatment.…”

“…The treatment was well tolerated by the patients. In a two-armed study, Ikushima et al compared the efficiency and tolerability of weekly administered cisplatin 30 mg/m 2 and 40 mg/m 2 in the radiochemotherapy for tumors of the uterine cervix [9]. The trial unambiguously proved the efficiency of the 30-mg/m 2 arm and -thanks to less side effects -the tolerability.…”

“…The first concerns the feasibility of using the standard chemotherapy of weekly 40 mg/m 2 cisplatin concurrently with radiotherapy. There have been several reports that Japanese cervical cancer patients frequently experienced severe toxicities, and investigators concluded that CCRT using weekly 40 mg/m 2 cisplatin may not be feasible for Japanese patients [2,3]. The second is that there are limited data on CCRT using high-dose-rate intracavitary brachytherapy (HDR-ICBT) [4,5].…”
