Despite the wealth of literature examining long term outcomes of preterm low birthweight children, few studies have directly assessed the developmental impact of being born full term but small for gestational age (SGA). We aim to determine whether (i) being SGA increases preschool behavioural problems and (ii) other risk factors operate differently in SGA and appropriate for gestational age (AGA) controls. 550 New Zealand European mothers and their 3.5 year old children participated in this study. All children were born at full term (> 37 weeks' gestation) and approximately half were SGA (≤ sex specific 10 th percentile for gestation) the remainder were AGA controls. Extensive data were collected at the child's birth, 1 year and 3.5 years. Behavioural problems were measured when children were 3.5 years, using the Strengths and Difficulties Questionnaire (SDQ). Multiple regression analyses were used to examine the associations between risk factors and behavioural problems; statistical weighting was used for analyses of the total study group. There was no significant difference in behavioural problems between SGA and AGA groups. In the total sample the significant predictors of behavioural problems included: mothers' school leaving age; smoking during pregnancy; maternal alcohol use during pregnancy; and absence of the father.Predictors of behavioural problems were found to be the same for SGA and AGA groups.These results do not support the view that SGA is a risk for behavioural preschool difficulties or that SGA children are sensitized to risks known to be associated with such difficulties in the preschool years.

ResumoA literatura demonstra que sofrer violência doméstica na infância e juventude é um risco para o desenvolvimento das psicopatologias. Este estudo utilizou o Strenghts and Diffi culties Questionnarie (SDQ) e encontrou prevalência de 65,5% de possibilidades de problemas de saúde mental em crianças e adolescentes (4-16 anos), vítimas de violência doméstica, acompanhados em Serviços de Referência, todos residentes em um município brasileiro. O resultado encontrado foi considerado elevado, se comparado à população geral. A violência doméstica foi observada como fator de risco para problemas de saúde mental nas crianças e adolescentes estudados, agravada pelo uso preocupante de bebida alcoólica pelo responsável, ou este estar fora do processo produtivo. Não foram encontrados fatores de proteção efetivos, principalmente em relação à rede de proteção. Esses resultados sinalizam a necessidade de implementação de mecanismos que avaliem a efetividade de programas específi cos nas áreas de saúde mental e violência doméstica. AbstractLiterature shows that experiencing domestic violence in childhood and young adulthood is a risk for the development of psychopathology. This study used the Strengths and Diffi culties Questionnaire (SDQ) and found a prevalence of 65.5% of possibility of mental health problems in children and adolescents (4-16 years old) who were victims of domestic violence followed in Referral Services in a Brazilian city. The result found was considered high when compared to the general population. Domestic violence was observed as a risk factor for mental health problems among the studied children and adolescents, aggravated by caregivers' alcoholism or unemployment. No effective protective factors were found, mainly regarding the protection network. These results indicate the need for reassessing the effectiveness of specifi c programs in the areas of mental health and domestic violence.

Severe physical punishment: risk of mental health problems for poor urban children in Brazil

Objective To examine the relationship between specific types of child mental health problems and severe physical punishment, in combination with other important known risk factors. Methods We conducted a cross-sectional study in Embu, São Paulo, Brazil, as the Brazilian component of a multicountry survey on abuse in the family environment. From a probabilistic sample of clusters that included all eligible households (women aged 15-49 years with a son or daughter < 18 years of age), we randomly selected one mother-child pair per household (n = 813; attrition rate: 17.6%). This study focused on children aged 6-17 years (n = 480). Child Behaviour Checklist CBCL/6-18 was used to identify children with internalizing problems only, externalizing problems only, and both internalizing and externalizing problems (comorbidity). Severe physical punishment was defined as being hit with an object, being kicked, choked, smothered, burnt, scalded, branded, beaten or threatened with a weapon. We examined other potential correlates from four domains: child (gender, age, ever witnessing marital violence); mother (education, unemployment, anxiety or depression, marital violence); father (absence, drunkenness); and family (socioeconomic status). The WHO Self-Reporting Questionnaire (SRQ-20) was used to identify maternal anxiety or depression (score > 7). Backward logistic regression analysis identified independent correlates and significant interactions. Findings Multivariate modelling showed that severe punishment was an independent correlate of comorbid internalizing and externalizing problems but was not associated with internalizing problems only. It increased the risk of externalizing problems alone only for children and adolescents not exposed to maternal anxiety or depression. Maternal anxiety or depression increased the risk only for children or adolescents not exposed to severe punishment. Conclusion Severe punishment may be related to child mental health problems, with the mechanism depending on the type of problem. Its influence persists in the presence of family stressors such as the father's absence and maternal anxiety or depression.Une traduction en français de ce résumé figure à la fin de l'article.
