‘Even though I Am Blind, I Am Still Human!’: the Neglect of Adolescents with Disabilities’ Human Rights in Conflict-Affected Contexts

Drawing on qualitative research undertaken with adolescents with disabilities from refugee and host communities in Jordan and the State of Palestine, this article critically interrogates the framing of child neglect, which to date has situated the state as a protector rather than a perpetrator, the narrow understanding of adolescent needs and the responsibility of international actors for ensuring that the full range of human rights of adolescents with disabilities is supported. We frame our findings on adolescent neglect through a multidimensional capabilities lens and argue that although both adolescence as a distinct lifecycle stage and the rights of persons with disabilities have moved up the development agenda, adolescents with disabilities remain largely invisible, and especially so in conflict-affected contexts. Our findings highlight that adolescents with disabilities have limited access to schooling, skills building for economic empowerment and healthcare, due to accessibility challenges, cost and highly limited specialist provisioning. Moreover, adolescents with disabilities also have very little access to psychosocial support or opportunities to develop the independence which is a hallmark of adolescence and critical for successful transitions into early adulthood. Rather than working to meet those needs, the government and UN agencies tasked with provisioning in conflict-affected areas continue to miss opportunities to link young people with disabilities to existent services and tend to rely on NGOs to deliver smallscale, time-bound programming rather than assuming responsibility for appropriate programming at scale.

Ongoing Pediatric Health Care for the Child Who Has Been Maltreated

Pediatricians provide continuous medical care and anticipatory guidance for children who have been reported to state child protection agencies, including tribal child protection agencies, because of suspected child maltreatment. Because families may continue their relationships with their pediatricians after these reports, these primary care providers are in a unique position to recognize and manage the physical, developmental, academic, and emotional consequences of maltreatment and exposure to childhood adversity. Substantial information is available to optimize follow-up medical care of maltreated children. This new clinical report will provide guidance to pediatricians about how they can best oversee and foster the optimal physical health, growth, and development of children who have been maltreated and remain in the care of their biological family or are returned to their care by Child Protective Services agencies. The report describes the pediatrician' s role in helping to strengthen families' and caregivers' capabilities and competencies and in promoting and maximizing high-quality services for their families in their community. Pediatricians should refer to other reports and policies from the American Academy of Pediatrics for more information about the emotional and behavioral consequences of child maltreatment and the treatment of these consequences. Pediatricians provide medical care and anticipatory guidance for children who have been maltreated. Because as many as 25% of the child population has experienced some form of maltreatment, medical encounters in a pediatric practice with maltreated children are not uncommon. 1-3 Although only a small proportion of children who have been maltreated are investigated by Child Protective Services (CPS), each year, state CPS agencies determine that approximately 700 000 children have been victims of child maltreatment. 4 Approximately 75% of these children are neglected, and about 17% are physically abused; many children suffer multiple forms of maltreatment. El objetivo de esta investigación fue determinar las opiniones que tiene el personal de salud de un hospital de tercer nivel sobre el maltrato infantil. El estudio fue observacional, descriptivo, temporalmente prospectivo, con muestreo no probabilístico de casos consecutivos. Se encuestó a personal de salud que prestaba servicios en el Hospital de Clínicas de la Universidad Nacional de Asunción, por medio de un cuestionario estandarizado. Se entrevistó a 77 profesionales. En lo que hace a los resultados acerca de las creencias sobre los hijos, las opciones “no mostrarle afecto” y “no atender sus necesidades básicas” fueron significativamente más graves para las mujeres. En cuanto a las creencias relacionadas con los padres, los comportamientos más observados en la vida cotidiana fueron “gritos” y “amenazas”. En cuanto a la gravedad y frecuencia de la conducta, se observó que el mayor acuerdo estuvo en que “los padres que usan drogas tienen más probabilidades de maltratar a sus hijos”. Para hacer frente a la problemática del maltrato infantil, desde el ámbito de la salud pública, es fundamental conocer si los profesionales tienen las herramientas y conocimientos suficientes para abordar esta situación, y establecer planes para mejorar dichos conocimientos. En Paraguay, el maltrato infantil no es infrecuente. La negligencia o abandono es el maltrato más frecuente, seguido por abuso sexual.
