background Patellofemoral pain (PFP) is prevalent in adolescence and adulthood and often persists. In contrast to other persistent musculoskeletal conditions, for which non-physical, psychological features are implicated, PFP remains largely conceptualised in mechanical terms. Aims To (1) identify whether the psychological characteristics of individuals with PFP differs from asymptomatic controls and (2) evaluate the correlations between psychological characteristics and PFP severity. study design Systematic review Methods A systematic review of the literature was conducted according to PRISMA guidelines. The Epidemiological Appraisal Instrument was used to evaluate quality. Studies measuring psychological constructs with patient-reported measures were included. Standardised mean differences were calculated and supported by narrative synthesis. results Twenty-five studies were eligible. Quality results ranged from 28.3% to 61.7%. Psychological constructs were reported under four groupings: mental health, cognitive factors, behavioural factors and other factors. There is limited evidence of mental health and cognitive differences in some individuals with PFP. Features demonstrating linear correlations with pain and physical function included anxiety/depression, catastrophising, praying and hoping and pain-related fear. conclusions Anxiety, depression, catastrophising and fear of movement may be elevated in individuals with PFP and correlate with pain and reduced physical function. These results derive from a limited number of studies. Future research should aim to evaluate if and how psychological factors contribute to PFP. clinical relevance Patients are likely to benefit from clinician vigilance to the presence of psychological factors.

KEYWORDSanterior knee pain; functional activity; isokinetic; kinematics Abstract Weakness of hip external rotator muscles may cause excessive hip internal rotation during functional activity and it is associated with the development of patellofemoral pain (PFP). The aetiology of PFP is multifactorial in nature. Therefore, routine enrolment of patients into a hip-strengthening programme may not be effective and cost-effective. The present study examined the effect of fatigue-induced hip internal rotation during functional activity. Twenty subjects with PFP were asked to descend a step in barefoot before and after an isokinetic exercise protocol to induce muscle fatigue of hip external rotators. The hip motion was captured by eight high-speed cameras and analysed by a three-dimensional motion analysis system. The level of PFP during step descent and their usual PFP level were measured by validated instruments. Patients with PFP presented with more pronounced hip pathomechanics (p < 0.001) and greater level of PFP (p Z 0.001) after muscle fatigue. The pain score after muscle fatigue during step descent better reflects the usual functional disturbance associated with PFP (r s Z À0.655, p Z 0.002). Physical examination with induced muscle fatigue may be helpful in clinical decisions on the management programme for patients with PFP, which are related to altered hip mechanics.

Objective: to carry out linguistic and cultural adaptation and validation of the Kujala questionnaire that is today one of the most popular orthopedic tools used to evaluate the severity of pain in the anterior part of knee joint.Subjects and methods. In accordance with protocols on this type of investigations, the questionnaire was first translated directly and then back. Furthermore, an intermediate Russian-language version of the Kujala questionnaire was tested in 15 patients, followed by error correction and approval of its final Russian-language version. A study group included 50 patients who complained of obvious pain in the anterior part of knee joint and answered the questionnaire twice (for its test-retest reliability) every two or three days.Results and discussion. The intraclass correlation coefficient (ICC) equal to 0.948 (95% confidence interval, 0.025–0.967) indicated a high test-retest reliability of the Russian-language version of the Kujala questionnaire. Cronbach's alpha coefficient of 0.956 corresponded to a high level of internal consistency, which also suggested that the proposed version of the questionnaire had a high reliability. The criterion-related validity assessment, by calculating the Spearman correlation coefficient between the results of primary and repeated surveys using the Russian-language version of the Kujala questionnaire, as well as between the Russian- language versions of the Kujala questionnaire and the SF-36, showed their high correlation.Conclusion. The findings indicate that the Russian-language version of the Kujala questionnaire is a valid and reliable tool for subjective assessment of the severity of pain in the anterior part of knee joint.
